Systemic lupus erythematosus (SLE), also known simply as lupus, is an autoimmune disease in which the body's immune system mistakenly attacks healthy tissue in many parts of the body.[1] Symptoms vary between people and may be mild to severe.[1] Common symptoms include painful and swollen joints, fever, chest pain, hair loss, mouth ulcers, swollen lymph nodes, feeling tired, and a red rash which is most commonly on the face.[1] Often there are periods of illness, called flares, and periods of remission during which there are few symptoms.[1]

Systemic lupus erythematosus (SLE) is a complex multisystemic autoimmune disease resulting, oftentimes, in irreversible damage, diminished quality of life and reduced life expectancy.1–3 Genetic and environmental factors play important roles in its pathogenesis.4–8 Disease manifestations and severity vary according to the patients’ racial/ethnic background and socioeconomic status (SES).1 9 10 Data from Grupo Latino Americano de Estudio del Lupus (GLADEL), Lupus in Minorities: Nature vs Nurture (LUMINA) and the Lupus Family Registry and Repository cohorts have demonstrated that Latin American and North American Mestizo patients (mixed Amerindian and European ancestry), African descendants and Native Americans develop lupus earlier11 12 although diagnostic delays may occur.1 They also experience more severe disease, have higher disease activity levels,1 accrue more organ damage2 and have higher mortality rates,1 succumbing mainly to disease activity and/or infections.1 3 13–15

The complement system is the name of a group of blood proteins that help fight infection. Complement levels, as the name implies, measure the amount and/or activity of those proteins. Working within the immune system, the proteins also play a role in the development of inflammation. In some forms of lupus, complement proteins are consumed (used up) by the autoimmune response. A decrease in complement levels can point toward lupus nephritis, lupus nephritis, kidney inflammation. Normalization of complement levels can indicate a favorable response to treatment.
Articular cartilage is the highly specialized connective tissue of diarthrodial joints. Its principal function is to provide a smooth, lubricated surface for articulation and to facilitate the transmission of loads with a low frictional coefficient. Articular cartilage is devoid of blood vessels, lymphatics, and nerves and is subject to a harsh biomechanical environment. Most important, articular cartilage has a limited capacity for intrinsic healing and repair. In this regard, the preservation and health of articular cartilage are paramount to joint health.
In a study published in 2015, patients with SLE were referred for nutrition counseling with a registered dietician (RD), and 41 of 71 referrals participated in the sessions.8 At the end of the 6-month period, the patients who received nutrition counseling were more likely to have lost weight; decreased their intake of foods high in fat, sodium, and calories; and increased their consumption of fruits, vegetables, fiber, and fish.
For each of the subheadings listed below, the panel considered interventions based on experience, availability, affordability and a stepwise therapeutic approach of the different alternatives. Standard of care (SOC) was defined as the use of hydroxychloroquine (HCQ) and, if clinically indicated, low-dose glucocorticoids (GC) (prednisone ≤7.5 mg or equivalent for the shortest time).24 Chloroquine remains an alternative for some of the Latin American countries where HCQ is not available and careful monitoring of eye side effect is recommended. When Griffiths et al compared the corticosteroid-sparing effect of cyclosporine with azathioprine in patients with severe SLE, they concluded that azathioprine may be considered first-line therapy, whereas cyclosporine requires close monitoring of blood pressure and serum creatinine. However, the investigators noted that in patients who are unable to tolerate azathioprine, cyclosporine may be considered. [136]
Avoid calcium supplements, however, which Johns Hopkins researchers have found to potentially increase the risk of heart damage and arterial plaque buildup. “Due to the risk of accelerated atherosclerosis in lupus, we no longer recommend calcium supplementation and encourage a diet rich in calcium instead,” noted George Stojan, MD, a rheumatologist and assistant professor of medicine at Johns Hopkins.

Nutrients that are important for managing lupus, such as fiber and antioxidants, seem to have the most beneficial effects when consumed from real food rather than from supplements.  What type of foods are included in a lupus diet? These include healthy fats, plenty of fresh veggies and fruits, and probiotic foods. Considering the fact that lupus can increase your risk for other chronic health problems (for example, women with lupus have a five- to tenfold higher risk for heart disease than the general population!), a nutrient-rich lupus diet can have far-reaching protective effects.


The principal receptors on animal cells for binding most extracellular matrix proteins—including collagens, fibronectin, and laminins—are the integrins. Integrins, like other cell adhesion molecules, differ from cell-surface receptors for hormones and for other extracellular soluble signal molecules in that they usually bind their ligand with lower affinity and are usually present at about tenfold to a hundredfold higher concentration on the cell surface. If the binding were too tight, cells would presumably become irreversibly glued to the matrix and would be unable to move—a problem that does not arise if attachment depends on large numbers of weak adhesions. This is an example of the “Velcro principle” mentioned earlier. Like other transmembrane cell adhesion proteins, however, integrins do more than just attach a cell to its surroundings. They also activate intracellular signaling pathways that communicate to the cell the character of the extracellular matrix that is bound.

The aim of this review is to provide an up-to-date overview of treatment approaches for systemic lupus erythematosus (SLE), highlighting the multiplicity and heterogeneity of clinical symptoms that underlie therapeutic decisions. Discussion will focus on the spectrum of currently available therapies, their mechanisms and associated side-effects. Finally, recent developments with biologic treatments including rituximab, epratuzumab, tumor necrosis factor (TNF) inhibitors, and belimumab, will be discussed.
Because some treatments may cause harmful side effects, it is important to report any new symptoms to the doctor promptly. It is also important not to stop or change treatments without talking to the doctor first. In addition to medications for lupus itself, in many cases it may be necessary to take additional medications to treat problems related to lupus such as high cholesterol, high blood pressure, or infection.
Kidney involvement in people with lupus is potentially life threatening and may occur in up to half of lupus patients. Kidney problems may become apparent when lupus patients feel ill with arthritis, have a rash, fever and weight loss. Less often, kidney disease may occur when there are no other symptoms of lupus. Kidney disease itself usually does not produce symptoms until it is in the advanced stages. It is important that kidney disease be diagnosed early and treated appropriately. The earliest signs of kidney disease are apparent from a urinalysis.
Steroids . Steroid creams can be applied directly to rashes. The use of creams is usually safe and effective, especially for mild rashes. The use of steroid creams or tablets in low doses can be effective for mild or moderate features of lupus. Steroids also can be used in higher doses when internal organs are threatened. Unfortunately, high doses also are most likely to produce side effects.

Why the test is used: Between 75% and 90% of people with lupus have a positive anti-dsDNA test. Also, the test is very specific for lupus. Therefore, a positive test can be useful in confirming a diagnosis. For many people, the titer, or level, of the antibodies rises as the disease becomes more active. So, doctors can also use it to help measure disease activity. Also, the presence of anti-dsDNA indicates a greater risk of lupus nephritis, a kidney inflammation that occurs with lupus. So a positive test can alert doctors to the need to monitor the kidneys.
Acute cutaneous: This is the type of skin flare that occurs when your SLE is active. Lesions associated with acute cutaneous lupus appear as flattened areas of red skin on the face, reminiscent of a sunburn—the telltale butterfly rash. These lesions can appear on the arms, legs, and body, and are photosensitive. Though the lesions may discolor the skin, they don't scar. Lesions typically appear during a flare or after sun exposure.
Lupus can cause problems with the blood, too, including anemia, or low red blood cell count. Anemia can cause symptoms such as weakness and fatigue. (14) Thrombocytopenia is another blood disorder that may develop, resulting in lower platelet counts. (Platelets are the blood cells that help the blood clot.) Symptoms of thrombocytopenia can include bruising easily, nosebleeds, and petechiae, when the blood appears as red pinpoints under the skin. (15)

A. Like Gomez, people with lupus often begin chemotherapy, which helps to suppress the immune system. Gomez has said that she is in remission, which means her disease is not causing her any symptoms. With luck, these remissions can last for years. But about 25% of people with lupus a year experience a "flare," in which symptoms recur. To keep the disease under control, people with lupus need to be treated for the rest of their lives. Most take a drug called hydroxychloroquine, which is also used to fight malaria. People also usually take an immune-suppressing drug, Gilkeson said.

Neuropsychiatric syndromes can result when SLE affects the central or peripheral nervous system. The American College of Rheumatology defines 19 neuropsychiatric syndromes in systemic lupus erythematosus.[30] The diagnosis of neuropsychiatric syndromes concurrent with SLE (now termed as NPSLE),[31] is one of the most difficult challenges in medicine, because it can involve so many different patterns of symptoms, some of which may be mistaken for signs of infectious disease or stroke.[32]
Other diseases and conditions that can accompany lupus include fibromyalgia, coronary heart disease, nonbacterial valvular heart disease, pancreatitis, esophagus disease with difficulty swallowing (dysphagia), swollen lymph nodes (lymphadenopathy), liver disease (lupoid hepatitis), infections, and a tendency to spontaneous blood clotting and thrombosis.
Cells are the basic building blocks of all living things. The human body is composed of trillions of cells. They provide structure for the body, take in nutrients from food, convert those nutrients into energy, and carry out specialized functions. Cells also contain the body’s hereditary material and can make copies of themselves. When someone has lupus, the immune system can’t tell the difference between the body’s healthy cells and bacteria and viruses, so the antibodies attack the body’s healthy cells.
The male hormone DHEA (dehydroepiandrosterone), produced in the adrenals, seems to help and may reduce the need for prednisone. Although DHEA is available over-the-counter, don’t take it without medical supervision. It presents an increased risk of heart attack and breast and prostate cancer so it is vital that a physician monitor anyone taking it for lupus. Furthermore, over-the-counter brands of DHEA may not be as reliable as prescription forms.
With variants known as discoid lupus, subacute cutaneous lupus, and systemic lupus erythematosus, lupus is one of several disorders of the immune system considered “autoimmune” in nature. These diseases occur when the immune system malfunctions and turns its infection-defense capabilities against the body, producing antibodies against healthy cells and tissues. These antibodies promote chronic inflammation and can damage organs and tissues. In lupus, these antibodies are known as antinuclear antibodies (ANA) because they target parts of the cell’s nucleus. Experts don’t yet fully understand all of the factors and triggers that cause inflammation and tissue damage in lupus, and research is ongoing.
Lupus is often missed or misdiagnosed because the symptoms are vague and can match those of other conditions. Generally, a doctor will review your medical history and your family history, and look for signs of systemic inflammation. Because lupus can involve the internal and external organs, a doctor will rely on observation as well as laboratory testing in order to make a lupus diagnosis. There is no one test for lupus–generally, many different criteria will need to come together, and it can take years to reach a diagnosis.
Conventional medicine does not look at the body as a whole, instead viewing it in terms of isolated systems, with a separate doctor for each one. Generally, lupus patients are under the care of a rheumatologist and a doctor who specializes in the area in which they are experiencing symptoms–for example, a nephrologist for your kidneys, and a dermatologist for your skin.
Approval for SC belimumab was based on the BLISS-SC phase III study (n=839), which documented reduction in disease activity at week 52 in patients receiving belimumab plus standard of care, compared with those receiving placebo plus standard of care. SRI response with belimumab versus placebo was 61.4% vs 48.4%, respectively (P = 0.0006). In the belimumab group, both time to and risk of severe flare were improved (median 171 days vs 118 days; P = 0.0004), and more patients were able to reduce their corticosteroid dosage by ≥25% (to ≤7.5 mg/day) during weeks 40-52 (18.2% vs 11.9%; P = 0.0732), compared with placebo. [163]

While the genetics of SLE are not very well understood, there is growing evidence for the involvement of specific genes in this complex autoimmune disease. Part of the complexity of this disease is due to the effects of both environment and genetics factors that may contribute to its development.[49] Further compounding our understanding of the etiology of the disease is the involvement of several organ systems.[50] Genetic studies of the rates of disease in families supports the genetic basis of this disease with a heritability of >66%.[51] Identical (monozygotic) twins were found to share susceptibility to the disease at >35% rate compared to fraternal (dizygotic) twins and other full siblings who only showed a 2–5% concordance in shared inheritance.[51]


Whole foods, especially the kinds high in probiotics, antioxidants and prebiotic fiber, can lower inflammation by increasing “good bacteria” in the gut, which help with absorption and defending against toxins or bad bacteria. High-antioxidant foods also have anti-aging effects even for those without lupus or another immune disorder because they fight free radical damage that degenerates cells and tissues.
The most serious health risks are cardiovascular disease, kidney disease and stroke. Specifically, people with lupus are at increased risk for atherosclerosis (hardening of the arteries). In some people, inflammation can occur in the heart itself (myocarditis and endocarditis) or the membrane that surrounds it. Endocarditis can damage heart valves, which can result in heart murmurs. When the disease affects the kidneys, patients generally require intensive drug treatment to prevent permanent damage. Lupus also may attack the brain or central nervous system, which can cause seizures or stroke.
This diet is not intended for weight loss (although that can be a side effect). The anti-inflammatory diet is intended to provide steady energy, plenty of vitamins and minerals, and the essential fatty acids needed to maintain optimum health. You could look at this more like an eating plan for life as opposed to a diet per se. It is based on the general concept that eating to avoid inflammation promotes better health and can ward off diseases. According to Dr. Andrew Weil, the Harvard trained natural and preventative medicine physician (as seen on Oprah, and the Dr. Oz show,) there is clear evidence to support that inflammation can be very damaging to the body and he therefore openly supports the Anti-Inflammatory Diet. “We all know inflammation on the surface of the body as local redness, heat, swelling and pain. It is the cornerstone of the body’s healing response, bringing more nourishment and more immune activity to a site of injury or infection. But when inflammation persists or serves no purpose, it damages the body and causes illness. Stress, lack of exercise, genetic predisposition, and exposure to toxins (like secondhand tobacco smoke) can all contribute to such chronic inflammation, but dietary choices play a big role as well.” Both he and Barry Sears, MD, the author of the well-known Zone Diet both agree that this diet can have significant positive results on many diseases. Here are the basics of the anti-inflammatory diet (all versions vary, but this is the general proposal for all:
The body’s tolerance of the antigens present on its own cells, i.e., autoantigens or self-antigens. It is theorized that autoreactive T lymphocytes are destroyed in the thymus by negative selection or in peripheral blood. Autoreactive T cells that escape destruction in the thymus may become tolerant because they are exposed to thousands of autoantigens as they circulate in the blood.
Dermatomyositis. Acute onset of confluent macular erythema in a periorbital and malar distribution (involving the cheeks and extending over the nasal bridge), with extension to the chin in a female with juvenile dermatomyositis. Note the perioral sparing. In some patients, there may be more extensive involvement of the face, including the perioral region, forehead, lateral face, and ears. In contrast to SLE , in dermatomyositis with malar erythema, the nasolabial folds are often not spared.
Nonsteroidal anti-inflammatory drugs (NSAIDs): NSAIDs decrease joint swelling, joint pain, fever, and inflammation of the heart and lung linings. These drugs include ibuprofen (brand names Motrin, Advil) and naproxen (Naprosyn, Aleve). Some of these NSAIDs can cause serious side effects like stomach bleeding or kidney damage. Always check with your doctor before taking any medications that are over the counter (without a prescription) for your lupus.
