Based on the identified evidence the panel concluded that compared with GCs alone, the addition of other IS (CYC, MMF or TAC) is associated with significant benefits, higher remission rates and lower progression rates to end-stage renal disease (ESRD). Head-to-head comparisons between MMF, TAC and high-dose CYC showed that MMF and TAC are associated with less adverse effects than high-dose CYC. Between low and high-dose CYC the balance favours the former because of better safety profile and comparable efficacy, although this conclusion is based on one trial that included predominantly Caucasians. RTX did not provide additional benefits when combined with MMF.
A. Lupus is a chronic disease in which a person's body is attacked by the immune system, which normally fights infections and foreign invaders, such as viruses and bacteria, said Gilkeson, a professor of medicine at the Medical University of South Carolina in Charleston. Lupus can cause a variety of symptoms, including severe fatigue, headaches, painful or swollen joints, fever, swelling in the hands or ankles, a butterfly-shaped rash across the nose and cheeks, sensitivity to light, mouth and nose ulcers, anemia and hair loss.
Lupus is treated by internal medicine subspecialists called rheumatologists. Depending on whether or not specific organs are targeted, other health specialists who can be involved in the care of patients with lupus include dermatologists, nephrologists, hematologists, cardiologists, pulmonologists, and neurologists. It's not uncommon that a team of such physicians is coordinated by the treating rheumatologist together with the primary care doctor.

Symptoms vary from person to person, but the typical lupus patient is a young woman experiencing fever, swollen lymph nodes (glands), butterfly-shaped rash on her face, arthritis of the fingers, wrists or other small joints, hair loss, chest pain and protein in the urine. Symptoms usually begin in only one or two areas of the body, but more may develop over time. The most common signs and symptoms of lupus are:


The erythrocyte sedimentation rate (ESR) test is a blood test that measures inflammation in your body and is used to help diagnose conditions associated with acute and chronic inflammation, including lupus. It is usually used in conjunction with other tests, as the test itself is nonspecific. In other words, it can detect increases in inflammation, but it doesn't pinpoint where the inflammation is or point to a specific disease. Other conditions can affect outcomes of the test as well. The test is one that is usually conducted several times over a certain period to measure changes in inflammation.

CAD happens when the arteries that supply blood to heart muscle become hardened and narrowed. This is due to the buildup of cholesterol and other material, called plaque, on their inner walls. This buildup is called atherosclerosis. As it grows, less blood can flow through the arteries. As a result, the heart muscle can’t get the blood or oxygen it needs. This can lead to chest pain (angina) or a heart attack. Most heart attacks happen when a blood clot suddenly cuts off the hearts’ blood supply, causing permanent heart damage.
Recommendations are applicable to patients showing partial or total remission after induction therapy aiming at sustaining renal remission, preventing relapses and achieving the best long-term outcome. The following interventions were considered: (1) AZA; (2) MMF; (3) CYC; (4) TAC; and (5) CsA (online supplementary tables S1.1.1.7, S1.1.2.1, S1.1.2.2, S1.2.1, S1.2.3, S1.2.4, S1.2.5, S1.2.6, S1.2.7).

The rate of SLE varies between countries, ethnicity, and sex, and changes over time.[95] In the United States, one estimate of the rate of SLE is 53 per 100,000;[95] other estimates range from 322,000 to over 1 million.[96] In Northern Europe the rate is about 40 per 100,000 people.[97] SLE occurs more frequently and with greater severity among those of non-European descent.[96] That rate has been found to be as high as 159 per 100,000 among those of Afro-Caribbean descent.[95] Childhood-onset systemic lupus erythematosus generally presents between the ages of 3 and 15 and is four times more common in girls.[98]

Lupus affects people in many different ways, so there is not one diet which is guaranteed to work for everyone, but the Mediterranean diet (plenty of fruit and vegetables, grains, nuts and seeds, two portions of fish per week and small amounts of meat and dairy produce) is probably the simplest one to follow and is suitable for all the family as it is a pattern of healthy eating.

The EULAR recommendations indicate that in pregnant women with SLE, prednisolone, azathioprine, hydroxychloroquine (unnecessary discontinuation of hydroxychloroquine during pregnancy may result in lupus flares), and low-dose aspirin may be used. [61] Prednisone, prednisolone, and methylprednisolone are the corticosteroids of choice during pregnancy because of their minimal placental transfer. However, mycophenolate mofetil, cyclophosphamide, and methotrexate are strictly contraindicated. [61]
One common early symptom that can be indicative of lupus is a photosensitive rash, meaning a rash that develops in response to sun exposure, particularly on the face and upper arms, says Dr. Kramer. Other early symptoms are unexplained fever and pain, swelling, and stiffness of multiple joints. Complications such as inflammation of the lining surrounding the lungs or heart can also occur early on, he adds.

The first mechanism may arise genetically. Research indicates SLE may have a genetic link. SLE does run in families, but no single causal gene has been identified. Instead, multiple genes appear to influence a person's chance of developing lupus when triggered by environmental factors. HLA class I, class II, and class III genes are associated with SLE, but only classes I and II contribute independently to increased risk of SLE.[45] Other genes which contain risk variants for SLE are IRF5, PTPN22, STAT4,[46] CDKN1A,[47] ITGAM, BLK,[46] TNFSF4 and BANK1.[48] Some of the susceptibility genes may be population specific.[46]


An adverse event that results in death, is life-threatening, requires inpatient hospitalization or extends a current hospital stay, results in an ongoing or significant incapacity or interferes substantially with normal life functions, or causes a congenital anomaly or birth defect. Medical events that do not result in death, are not life-threatening, or do not require hospitalization may be considered serious adverse events if they put the participant in danger or require medical or surgical intervention to prevent one of the results listed above.

Lupus is a chronic autoimmune disease that causes the immune system to attack healthy tissue resulting in inflammation, particularly of the skin, joints, kidneys, heart, lungs, blood vessels and brain. It develops most commonly in women between the ages of 15-45, and occurs more often in African-American, Hispanics, Native Americans and Asians. Men can get lupus too. Lupus is not infectious or cancerous. People with lupus may have many different symptoms affecting various parts of the body. Some of the most common symptoms are extreme fatigue, painful or swollen joints (arthritis), unexplained fevers, skin rashes and kidney problems. Lupus is characterized by “flares” or periods of illness and remission. Warning signs of a flare can be increased fatigue, pain, rash, fever, abdominal discomfort, headache or dizziness. Learning how to recognize these signs can help people maintain better health and reduce or ward off a flare. Currently, there is no cure for lupus but it can be managed effectively with drugs, and most people with lupus lead an active, healthy life.


There have been several diet studies using omega-3 fatty acids in people who have lupus. A 2012 study looked at the eating habits of 114 SLE patients. They found that those who had a diet low in omega-3 fatty acids had worse lupus disease activity as well as higher levels of cholesterol and atherosclerosis (which can cause heart attacks and strokes). Therefore, it is important for people who have lupus to supplement their diet with foods rich in omega-3 fatty acids, olive oil, or supplements containing these oils. Not only may this possibly improve lupus disease activity, but it may also improve cholesterol levels, which could help to decrease the risk of getting heart attacks, strokes and blood clots.
The GRADE approach was followed in the process (http://www.gradeworkinggroup.org) answering the clinical questions voted most relevant by the panel. The description of the methodology followed to develop these guidelines has already been published.29 All authors listed in this manuscript have participated in planning, drafting, reviewing, final approval and are accountable for all aspects of the manuscript. No ethical approval was required by institutions. We present the final recommendations and their supporting information. Comments from three patients with SLE were also considered.

For arthritic symptoms, take a natural anti-inflammatory agent, containing ginger and turmeric. Get the right kind of regular exercise; swimming or water aerobics are best for those who have arthritis symptoms. Investigate traditional Chinese medicine and Ayurvedic medicine, both of which often do well with autoimmune conditions. Definitely try one or more mind/body therapies, such as hypnosis or interactive guided imagery.
It also is known that some women with systemic lupus erythematosus can experience worsening of their symptoms prior to their menstrual periods. This phenomenon, together with the female predominance of systemic lupus erythematosus, suggests that female hormones play an important role in the expression of SLE. This hormonal relationship is an active area of ongoing study by scientists.
That being said, many physicians support the following of any nutritional plans that are designed to fight inflammation and support the immune system. According to the Department of Health and Human Services and American Heart Association, chronic inflammation might cause diseases such as atherosclerosis, arthritis, osteoporosis, Alzheimer’s disease, food intolerances, diabetes, fibromyalgia, heart disease and in some cases even cancer. It also accelerates the aging process. Nutrition is a very powerful way to protect your cells from inflammation, thus the connection. Lupus, like any other auto-immune disease is different for each individual. While something may work for one person, it may not work for another. In general, it is a good idea for people with autoimmune disorders to discuss any major dietary changes with their doctor beforehand. We are writing this blog primarily in order to provide information and respond to the conversations occurring on our social media platforms with regards to these two diets. DHEA (dehydroepiandrosterone) has been helpful in reducing fatigue, improving thinking difficulties, and improving quality of life in people with SLE. Recent research indicates that DHEA diet supplementation has been shown to improve or stabilize signs and symptoms of SLE. DHEA is commonly available in health-food stores, pharmacies, and many groceries.
Corticosteroids also can cause or worsen osteoporosis, a disease in which bones become fragile and more likely to break. If you have osteoporosis, you should eat foods rich in calcium every day to help with bone growth. Examples are dark green, leafy vegetables (spinach, broccoli, collard greens), milk, cheese, and yogurt or calcium supplements that contain Vitamin D.

No single finding qualifies an individual as having SLE. Instead, the American College of Rheumatology (ACR) has devised certain classification criteria, and four or more of these criteria must be present for a classification of lupus. [The term “classification” is not synonymous with “diagnosis.” “Classification” means that reasonable certainty exists for the diagnosis of lupus for research purposes.] Although, these criteria are currently being updated, they are believed to be about 90% effective. The ACR criteria include malar rash; discoid rash; photosensitivity (development of a rash after sun exposure); oral or nasal ulcers; arthritis of multiple joints; serositis: (inflammation of the lining around the lungs or heart); kidney disease indicated by protein or casts in the urine; neurological disorders such as seizures and psychosis; and blood disorders such as hemolytic anemia, leukopenia, and lymphopenia. Other signs that are common but not included in the classification criteria are hair loss or breaking, especially around the forehead, and Raynaud’s Phenomenon, a two- or three-color change of the fingertips upon cold exposure.

Limited evidence suggests that supplementation may be clinically beneficial in SLE patients with low levels of vitamin D. In Mediterranean patients,  female patients who were not receiving supplemental vitamin D showed more fatigue and received more oral corticosteroids than those with normal levels of vitamin D. [109] In Australian patients, an increase in serum vitamin D levels was associated with reduced disease activity over time. [152]

Conventional medicine does not look at the body as a whole, instead viewing it in terms of isolated systems, with a separate doctor for each one. Generally, lupus patients are under the care of a rheumatologist and a doctor who specializes in the area in which they are experiencing symptoms–for example, a nephrologist for your kidneys, and a dermatologist for your skin.

While there’s no one dietary program that can cure or treat lupus for all patients, a healthy lupus diet can go a long way in preventing flare-ups and decreasing complications. Molly’s Fund for Fighting Lupus states that a healthy diet is needed to prevent nutrient deficiencies, maintain strength and energy, combat medication side effects, maintain a healthy weight, and protect the heart. (4)

Early mortality, within 5 years, is due to organ failure or overwhelming infections, both of which can be altered by early diagnosis and treatment. The mortality risk is fivefold when compared to the normal population in the late stages, which can be attributed to cardiovascular disease from accelerated atherosclerosis, the leading cause of death for people with SLE.[83] To reduce the potential for cardiovascular issues, high blood pressure and high cholesterol should be prevented or treated aggressively. Steroids should be used at the lowest dose for the shortest possible period, and other drugs that can reduce symptoms should be used whenever possible.[83]

According to the Lupus Foundation of America, approximately 1.5 million people in the U.S. have lupus. People of African, Asian, and Native American descent are more likely to develop lupus than are Caucasians. Although it can occur in both men and women, 90% of people diagnosed with the disease are women. Women of childbearing age (14 to 45 years old) are most often affected and as many as 1 in 250 people may develop lupus.

Foot pain may be caused by injuries (sprains, strains, bruises, and fractures), diseases (diabetes, Hansen disease, and gout), viruses, fungi, and bacteria (plantar warts and athlete's foot), or even ingrown toenails. Pain and tenderness may be accompanied by joint looseness, swelling, weakness, discoloration, and loss of function. Minor foot pain can usually be treated with rest, ice, compression, and elevation and OTC medications such as acetaminophen and ibuprofen. Severe pain should be treated by a medical professional.

Blood—hematologic disorder—hemolytic anemia (low red blood cell count), leukopenia (white blood cell count<4000/µl), lymphopenia (<1500/µl), or low platelet count (<100000/µl) in the absence of offending drug; sensitivity = 59%; specificity = 89%.[75] Hypocomplementemia is also seen, due to either consumption of C3[76] and C4 by immune complex-induced inflammation or to congenitally complement deficiency, which may predispose to SLE.
In some cases, your doctor may want to do a biopsy of the tissue of any organs that seem to be involved in your symptoms. This is usually your skin or kidney but could be another organ. The tissue can then be tested to see the amount of inflammation there is and how much damage your organ has sustained. Other tests can show if you have autoimmune antibodies and whether they're related to lupus or something else.

Systemic lupus erythematosus (SLE) is a complex multisystemic autoimmune disease resulting, oftentimes, in irreversible damage, diminished quality of life and reduced life expectancy.1–3 Genetic and environmental factors play important roles in its pathogenesis.4–8 Disease manifestations and severity vary according to the patients’ racial/ethnic background and socioeconomic status (SES).1 9 10 Data from Grupo Latino Americano de Estudio del Lupus (GLADEL), Lupus in Minorities: Nature vs Nurture (LUMINA) and the Lupus Family Registry and Repository cohorts have demonstrated that Latin American and North American Mestizo patients (mixed Amerindian and European ancestry), African descendants and Native Americans develop lupus earlier11 12 although diagnostic delays may occur.1 They also experience more severe disease, have higher disease activity levels,1 accrue more organ damage2 and have higher mortality rates,1 succumbing mainly to disease activity and/or infections.1 3 13–15
The history of SLE can be divided into three periods: classical, neoclassical, and modern. In each period, research and documentation advanced the understanding and diagnosis of SLE, leading to its classification as an autoimmune disease in 1851, and to the various diagnostic options and treatments now available to people with SLE. The advances made by medical science in the diagnosis and treatment of SLE have dramatically improved the life expectancy of a person diagnosed with SLE.[105]
Once remission is achieved, start maintenance therapy with azathioprine or mycophenolate mofetil (ie, use less potent agents relative to long-term cyclophosphamide). The ALMS maintenance trial also found that mycophenolate mofetil was superior to azathioprine in the maintenance of the renal response to treatment and in the prevention of relapse in patients with lupus nephritis. [134] In the MAINTAIN trial, there was a trend toward fewer renal flares in patients receiving mycophenolate mofetil than in those receiving azathioprine [135] ; however, these results did not reach statistical significance.
Useful medication for the disease was first found in 1894, when quinine was first reported as an effective therapy. Four years later, the use of salicylates in conjunction with quinine was noted to be of still greater benefit. This was the best available treatment until the middle of the twentieth century, when Hench discovered the efficacy of corticosteroids in the treatment of SLE.[121]
Decorin is a protein coded for by the DCN gene. This protein is a component of the extracellular matrix, which is the intricate lattice of proteins and other molecules that forms in the spaces between cells. Decorin is found in the extracellular matrix of a variety of connective tissues, including skin, tendon, bone, and cartilage. Connective tissues support the body’s joints and organs. Decorin is involved in the organization of proteins called collagens. Collagens strengthen and support connective tissues throughout the body. Collagens also play an important role in the cornea, which is the clear outer covering of the eye. Bundles of collagen called fibrils must be strictly organized for the cornea to be transparent. Decorin ensures that these collagen fibrils are uniformly sized and regularly spaced.
Individual test results can also vary from one visit to another, which can be very confusing.  A doctor will take into consideration a combination of factors as well as the test results when diagnosing lupus, and because of this, we encourage you inquire about the ANA and DNA testing, which doctors are often reluctant to give.  These two tests together can create a clearer picture of whether the diagnosis could be lupus.  Again, we must remind you that just because you test negative today, it does not mean that you won’t test positive tomorrow.

The American College of Rheumatology (ACR) established eleven criteria in 1982,[73] which were revised in 1997[74] as a classificatory instrument to operationalise the definition of SLE in clinical trials. They were not intended to be used to diagnose individuals and do not do well in that capacity. For the purpose of identifying people for clinical studies, a person has SLE if any 4 out of 11 symptoms are present simultaneously or serially on two separate occasions.
The prognosis for those with lupus often depends on the amount of organ involvement. In other words, is the disease targeting organs rather than skin and joints? Survival for lupus patients with central nervous system symptoms, major organ involvement, and/or kidney disease, is likely to be shorter than those with only skin and/or joint disease related to lupus. The most common cause of death associated with lupus is an infection due to immunosuppression, caused by medications used to manage the disease, especially early in ​the disease.
