In humans, a factor associated with response to combination therapy in a large clinical trial is the Thr92Ala polymorphism in the type 2 deiodinase gene (DIO2), wherein the subpopulation of patients with this genetic alteration had improved well-being and preference for combination therapy (7). This has led investigators to consider whether this polymorphism could confer a defect in the D2 pathway, but normal Thr92AlaD2 enzyme kinetics have been demonstrated (73). Only recently has the Thr92AlaD2 protein been found to have a longer half-life, ectopically localize in the Golgi apparatus, and significantly alter the genetic fingerprint in cultured cells and in the temporal pole of the human brain without evidence of reduced thyroid hormone signaling (74). The significance of these studies transcends the thyroid field—this polymorphism has now been associated with a constellation of diseases, including mental retardation, bipolar disorder, and low IQ (75). If hypothyroid carriers of Thr92AlaD2 benefit from alternate therapeutic strategies in replicate studies, then personalized medicine—based on genotype— may have a role.

Nature Throid or WPThyroid: This is a great alternative to Armour as it’s gluten-free (and as we discussed in the diet section, people with hypothyroidism are often gluten sensitive, intolerant or Celiac). I prefer this to Synthroid, too, because it’s not made in a lab and instead is a natural supplement (though it’s made from animal thyroids, the thyroid hormones are biologically similar to that found in humans.). Both Synthroid and Armour contain controversial inactive ingredients, including gluten, sugar, and colorants, whereas Nature Throid and WPThyroid do not contain any artificial colors or flavors, corn, peanut, rice, gluten, soy, yeast, egg, fish or shellfish.
The amount in broccoli, cabbage, and kale in a usual diet is considered of minimal risk. For example, there was no adverse effect on thyroid function from consuming five ounces of cooked Brussels sprouts every day for four weeks.5,6 One note of caution, if you have a thyroid disorder, it's important to realize that juicing concentrates the amount of thiocyanate, on the order of  2000 mcg per glass.7

This can lead to low T3 levels (58). In addition, elevated cortisol will cause thyroid hormone receptor insensitivity meaning that even if T3 levels are high enough, they may not be able to bind normally to receptor sites. And when this happens it doesn’t get into the cells.  Cortisol will also increase the production of reverse T3 (rT3), which is inactive (11).
If you have thyroid issues, then raw cruciferous vegetables may not be the best choice. You might want to skip the kale smoothies and salads, and eat your greens cooked instead. The reason is that the cruciferous vegetables contain goitrogens that may disrupt the thyroid if consumed in large quantities. Other cruciferous veggies include cabbage, Brussels sprouts, broccoli and cauliflower.
Goitrogens are substances found naturally in certain foods that can slow down the production of thyroid hormone—keep in mind, though, this phenomenon occurs typically in people with an underlying iodine deficiency (which is rare in the United States). Still, even for people without iodine deficiency, experts recommend not over-consuming goitrogenic foods.
Ashwagandha is an adaptogen herb that helps the body respond to stress, keeping hormone levels better in balance. Adaptogens helps lower cortisol and balance T4 levels. In fact, in clinical trials, supplementing with ashwagandha for eight weeks essentially worked as thyroxine treatment, helping hypothyroidism patients significantly increase thyroxine hormone levels and thus reduce the severity of the disorder. (13) Also, try other adaptogen herbs like rhodiola, licorice root, ginseng and holy basil, which have similar benefits.
Despite these successes, authors have questioned the efficacy of l-thyroxine monotherapy because about 10% to 15% of patients are dissatisfied as a result of residual symptoms of hypothyroidism (1, 2), including neurocognitive impairment (3), and about 15% of patients do not achieve normal serum triiodothyronine (T3) levels (4). Studies of several animal models indicate that maintaining normal serum T3 levels is a biological priority (5). Although the clinical significance of relatively low serum T3 in humans is not well-defined (1), evidence shows that elevating serum T3 through the administration of both l-thyroxine and l-triiodothyronine has benefited some patients (6, 7). Novel findings highlight the molecular mechanisms underlying the inability of l-thyroxine monotherapy to universally normalize measures of thyroid hormone signaling (8, 9), and new evidence may lay the foundation for a role of personalized medicine (10). Understanding the historical rationale for the trend toward l-thyroxine monotherapy allows us to identify scientific and clinical targets for future trials.

The thyroid uses iodine to convert T4 into freeT3. If you have hypothyroidism, you may not have an iodine deficiency per se, but your thyroid is almost certainly struggling in some way to get ahold of the iodine available to it and do what it needs to do with it. If the root cause is left unaddressed, simply increasing iodine is not always useful and at worst can be dangerous depending on how high you’re increasing your supplementation thinking if a little is good, then more will “solve” your problem.

It is hard for me to tell you what to do without a thorough health history…but I would start by following my anti-inflammatory nutrition plan as mentioned in this article.
