Although I’m a fan of getting what you need from real food whenever possible, herbs, supplements, and vitamins can still have a place in the thyroid diet plan. It’s hard to give tips here as many people over-medicate and only feel worse later. Hopefully these tips will help; they apply to most auto-immune conditions (This advice was given by my naturopath who focuses on hormonal and thyroid health.):
The thyroid gland needs iodine, therefore if you have an underactive thyroid gland you should increase the iodine intake in your diet. A well-balanced diet that includes iodine can help in alleviating the symptoms that you encounter with your condition. With the simplest intake of the foods mentioned above, you can greatly assist your thyroid in keeping up with your body's metabolism. (See Hypothyroidism and Iodine for more information)
Short of eating a few kelp salads, you probably don't have to worry about getting too much iodine from any other foods. In particular, dairy products are full of this nutrient (and in more manageable amounts), according to a 2012 research in the journal Nutrition Reviews. Part of the reason is because livestock are given iodine supplements and the milking process involves iodine-based cleaners. Plain, low-fat yogurt, or Greek yogurt is a good source—it can make up about 50% of your daily intake of iodine.

Although relatively low serum T3 levels could contribute to these residual manifestations, the higher serum T4:T3 ratio should also be considered. This has been well-established for 4 decades (28, 50, 59), but only recently has it been recognized as a relevant measure given that higher serum T4 levels will impair systemic T3 production via downregulation of a deiodinase pathway (9). Thus, some emphasis has recently been directed toward establishing the clinical significance of this ratio (1, 5).
Thyroid hormone replacement has been used for more than a century to treat hypothyroidism. Natural thyroid preparations (thyroid extract, desiccated thyroid, or thyroglobulin), which contain both thyroxine (T4) and triiodothyronine (T3), were the first pharmacologic treatments available and dominated the market for the better part of the 20th century. Dosages were adjusted to resolve symptoms and to normalize the basal metabolic rate and/or serum protein-bound iodine level, but thyrotoxic adverse effects were not uncommon. Two major developments in the 1970s led to a transition in clinical practice: 1) The development of the serum thyroid-stimulating hormone (TSH) radioimmunoassay led to the discovery that many patients were overtreated, resulting in a dramatic reduction in thyroid hormone replacement dosage, and 2) the identification of peripheral deiodinase-mediated T4-to-T3 conversion provided a physiologic means to justify l-thyroxine monotherapy, obviating concerns about inconsistencies with desiccated thyroid. Thereafter, l-thyroxine mono-therapy at doses to normalize the serum TSH became the standard of care. Since then, a subgroup of thyroid hormone–treated patients with residual symptoms of hypothyroidism despite normalization of the serum TSH has been identified. This has brought into question the inability of l-thyroxine monotherapy to universally normalize serum T3 levels. New research suggests mechanisms for the inadequacies of l-thyroxine monotherapy and highlights the possible role for personalized medicine based on deiodinase polymorphisms. Understanding the historical events that affected clinical practice trends provides invaluable insight into formulation of an approach to help all patients achieve clinical and biochemical euthyroidism.
Most people with hypothyroidism don’t need to steer clear of soy completely. But it’s a good idea to limit your consumption to a few servings a week, and to stick with minimally processed forms of soy like tempeh or miso. Foods containing processed soy protein isolates (like soy protein powder, soy protein bars, or soy-based meat analogs) tend to have a higher concentration of isoflavones, says Markley.
Like vitamin D deficiency, vitamin B12 deficiency is common in people with Hashimotos' disease. Due to its important role in red blood cell formation and nerve function, a deficiency in vitamin B12 may cause fatigue, loss of energy, and shortness of breath from anemia (low red blood cell count), as well as numbness and tingling from impaired neurologic function.
Many allergies and food intolerances today are from wheat and dairy products. This is because of the hybridized proteins of gluten and a1 casein. These proteins can lead to “leaky gut”, which in turn will cause inflammation of the thyroid and effect its function. If you can’t follow a grain-free diet, at least cut out gluten. Additionally, only consume dairy products that come from A2 cows, goat milk, or sheep milk. (2)
Another great source of selenium, nuts make a handy snack that you can take anywhere. They also go well in salads or stir-fries. Brazil nuts, macadamia nuts, and hazelnuts are all particularly high in selenium, which helps the thyroid function properly. With Brazil nuts, you only need to eat one or two; with other nuts, a small handful is enough to get your daily nutrients — and be sure to keep an eye on portion size, as nuts are also very high fat.
There is little mention of patients who did not respond symptomatically to treatment despite having normalization of their other measured variables, such as BMR or serum PBI, in the early clinical trials in the 1940s through 1960s. After the 1970s (38, 52), a new category of hypothyroid patient was recognized: the patient who received thyroid hormone replacement therapy, had normal serum TSH, and exhibited residual symptoms of hypothyroidism. Initially, such symptoms were largely dismissed as unrelated to the thyroid condition (62). Indeed, hypothyroidism is prevalent, and symptoms overlap with those of other common conditions, including menopause, depression, and chronic fatigue syndrome. Likewise, thyroid hormone had been administered for nonthyroid disorders, including obesity and psychiatric disease, for decades. Thus, it was difficult to assess whether patients with residual symptoms had been misdiagnosed. Residual symptoms were even attributed to nonadherence (63).
Seaweed has a high concentration of iodine, an essential nutrient for thyroid function. "Iodine is the precursor for the production of thyroid hormone," Dr. Dodell explains. Seaweed, packaged as nori, wakame, and dulse, can be used in sushi, soups, and salads. Another plus: Seaweed offers nutritional benefits of fiber, calcium, and vitamins A, B, C, E, and K.
Less common causes of hypothyroidism include congenital (birth) defects (one of the reasons for newborn screening is to check for failure of the pituitary gland to produce enough thyroid stimulating hormone, usually due to a benign pituitary tumor), and pregnancy. Some women can develop hypothyroidism during or immediately following pregnancy, often as a result of developing antibodies against their own thyroid tissue. This is dangerous for both the developing fetus and mother, and can lead to miscarriage, developmental abnormalities, premature delivery and an increased risk of preeclampsia – a potentially dangerous complication in the later stages of pregnancy.
Making dietary changes is your first line of defense in treating hypothyroidism. Many people with hypothyroidism experience crippling fatigue and brain fog, which prompts reaching for non-nutritional forms of energy like sugar and caffeine. I’ve dubbed these rascals the terrible twosome, as they can burn out your thyroid (and destabilize blood sugar).
Do you see “gluten-free”, “dairy-free” etc. popping up at the health stores today? This is because many people get off the “big five” (gluten, dairy, corn, eggs and soy) and experience significant changes. To find the culprits, I always start off with an Elimination Diet (I teach how to do the Elimination Diet at this free workshop) and this produces clear, unbiased results. You can also get a food intolerance test (not allergy; it’s different) done but they are far from accurate. Gluten is an infamous food for contributing to thyroid conditions, and eliminating it is key. However, often times, you would need to cut out more than just gluten if you wish to shape your diet for thyroid fitness.
Hypothyroidism Diet: One of the main causes of hypothyroidism is inflammation, so following an anti-inflammatory diet is key to improving your thyroid function. Likewise, ensuring your diet is rich in nutrient-dense foods, particularly iodine and selenium, will also help your thyroid produce sufficient levels of thyroid hormones. Some of the best foods to eat for your thyroid: wild-caught fish, coconut oil and ghee, seaweed, probiotic-rich foods like yogurt, sauerkraut and miso, sprouted whole grains and nuts, fiber-rich fruits and vegetables, bone broth, and plenty of good ole’ H20. Getting plenty of protein, healthy fat and fiber is of utmost importance when you have thyroid dysfunction.
Clara Schneider, MS, RD, RN, CDE, LDN, of Outer Banks Nutrition and author of numerous books, including The Everything Thyroid Diet Book, says, “The No. 1 priority is to get the thyroid disease under control. Clients need to have labs and medications addressed first. Weight changes are just not going to happen before all of that is under control.” She notes that Hashimoto’s typically occurs around menopause, which compounds the weight gain issue that many women experience during that time.
You may find that changing your diet will help. One suggestion is to reduce or eliminate sugar, limit fruit, dairy, and grains, and get your carbohydrates mainly from vegetables. Round out your diet with lean proteins and healthy fats. In addition, eating two to three meals a day, no snacks, and avoiding food after 8 p.m. seems to help balance hunger hormones and blood sugar—and promote fat burning.
