The normal values for the serum T4:T3 ratio are seldom discussed in the literature because measurement of serum T3 levels is not a recommended outcome in hypothyroidism (1). In a large study of approximately 3800 healthy individuals (4), the serum free T4:free T3 ratio was around 3, as opposed to a ratio of 4 in more than 1800 patients who had undergone thyroidectomy and were receiving l-thyroxine monotherapy. The corresponding serum free T4:free T3 ratio in patients continuing to receive desiccated thyroid is not well-defined, but the serum total T4:T3 ratio is known to be low (28, 50). In one study, the serum total T4:total T3 was about 40 in patients receiving desiccated thyroid and about 100 in those taking l-thyroxine monotherapy (60). Of course, this is affected by the timing of blood collection in relation to the timing of l-triiodothyronine administration, which is not commonly reported. Other key factors are the well-known poor reproducibility of the serum total T3 assay (61) and the interferences with direct measurement of free T3 (5).
It is doubtful that nutritional deficiencies are the sole cause of an underactive thyroid, but not having enough of these micronutrients and minerals can aggravate symptoms of low thyroid function. Increasing the intake of; vitamin D, iron, omega-3 fatty acids, selenium, zinc, copper, vitamin A, the B vitamins, and iodine can help in natural hypothyroid treatment.
Treatment for hyperthyroidism - Hyperthyroidism is the opposite of hypothyroidism; it is a condition in which the thyroid gland is over-producing the thyroid hormones thus causing a hormone imbalance in the body. Hyperthyroidism can be treated with radioactive iodine and/or anti-thyroid medications, both of which are meant to reduce and normalize the thyroid function. In some cases, these treatments can cause permanent hypothyroidism if too much medication is administered.
Congenital hypothyroidism - Congenital hypothyroidism means that a baby is born with the condition. This occurs when a baby is born without a thyroid or with only a partly formed one. Sometimes, the baby will have part or all of the thyroid in the wrong place in the body (called ectopic thyroid). In some babies, the thyroid cells or their hormones do not work right. All of these issues lead to lifelong hypothyroidism for that human being.
The development of TSH assays led to a dramatic reduction in thyroid hormone replacement dosage and the ability to diagnose with certainty milder forms of hypothyroidism. Discovery of peripheral T4-to-T3 conversion gave a physiologic means to justify l-thyroxine monotherapy. In combination with the concerns over consistency and safety of natural thyroid preparations, synthetic l-thyroxine was perceived as a more reliable therapy. These findings laid the foundation for the clinical practice trend away from natural thyroid preparations and toward l-thyroxine monotherapy at doses to normalize the serum TSH. Later, a subpopulation of patients with residual symptoms of hypothyroidism was recognized. It remains to be determined whether this is due to a trend of attributing nonspecific symptoms to minimal thyroid dysfunction, relatively low serum T3 levels and/or high T4:T3 ratio, or the role of Thr92AlaD2 polymorphism, and whether combination therapy with l-thyroxine plus l-triiodothyronine will be beneficial.
Studies have indicated that individuals with lower selenium levels are at higher risk for low T3 (50). Selenium has been shown to reduce rT3 levels and improve active T3 status (51). It also reduces anti-thyroid anti-body formation (52). Be sure to get selenomethionine which is the most effective form of selenium for reducing anti-body formation and improving thyroid function.
4. Mitochondrial Dysfunction: The mitochondria are the energy producing organelles in each cell of the body. They are extremely key in the bodies ability to handle oxidative stress. Dysfunction in the mitochondria leads to increased free radical and oxidative stress which creates immune alterations. This is a classic sign in Hashimoto’s autoimmune pathophysiology (22).

Thus, neither desiccated thyroid nor l-thyroxine monotherapy recreates a biochemical state of euthyroidism as defined by the serum T4:T3 ratio. l-Thyroxine and l-triiodothyronine combination therapy theoretically could be titrated to restore this measure, but such a method would be challenging because of the frequent dosing schedule needed to achieve stable serum T3 levels (5). New technology is needed to allow for steady delivery of l-thyroxine; only then would high-quality clinical trials best investigate the utility of the serum T4:T3 ratio as an outcome measure in hypothyroidism.
According to a Journal of the American Medical Association article, “when thyroid function is too low, the pituitary increases its output of TSH to stimulate the thyroid to work harder.” (4) Therefore, subclinical hypothyroidism — someone without obvious symptoms yet still with low thyroid function — represents a situation in which thyroid function is only mildly low, with the blood level of thyroxine near the normal range. Meanwhile, however, the blood level of TSH is elevated, and this indicates mild thyroid failure.
Exercise: Many patients with hypothyroidism have reported benefits from practicing a yoga pose called the Shoulder Stand, or sarvangasana, which increases circulation to the thyroid. Lie on your back with your arms (palms up) along your sides. Raise your legs at a right angle to the floor. Then raise your hips so your chin rests on your chest, supporting yourself with your elbows and upper arms on the floor and your hands on your hips. Keep your neck and shoulders flat on the floor, and stretch your torso and legs as straight as possible. Hold as long as comfortable, slowly working up to 5 minutes a day. Don’t do this pose if you are pregnant or menstruating, nor should you try it if you have glaucoma, sinus problems or high blood pressure. The Shoulder Stand may be more effective if you use visualization practices, imagining the thyroid gland waking up from a long period of inactivity and producing more thyroid hormone.
A neck lump or nodule is the most common symptom of thyroid cancer. You may feel a lump, notice one side of your neck appears to be different, or your doctor may find it during a routine examination. If the tumor is large, it may cause neck or facial pain, shortness of breath, difficulty swallowing, cough unrelated to a cold, hoarseness or voice change.
The thyroid gland is a 2-inch butterfly-shaped organ located at the front of the neck. Though the thyroid is small, it’s a major gland in the endocrine system and affects nearly every organ in the body. It regulates fat and carbohydrate metabolism, respiration, body temperature, brain development, cholesterol levels, the heart and nervous system, blood calcium levels, menstrual cycles, skin integrity, and more.1
This is huge topic, especially with women. You won’t be able to fix your thyroid without fixing the adrenals. The adrenals are also part of the endocrine system and fire up when you are stressed out. I recommend looking up adrenal fatigue symptoms to see if you have them. De-stressing by working with a therapist or life coach, getting into meditation, breathing, or positive thinking – or whatever works for you – is key.
Because it helps balance hormone levels, selenium can lower the risk for experiencing thyroid disorder during pregnancy (postpartum thyroiditis) and afterwards. Other studies have shown that when selenium deficiency is resolved through supplementation, patients experience on average 40 percent reduction in thyroid antibodies compared to a 10 percent increase in the placebo group. Selenomethionine is the preferred form of selenium supplementation as it is the form found naturally in food and about 90% of it is absorbed.
If you have thyroid issues, then raw cruciferous vegetables may not be the best choice. You might want to skip the kale smoothies and salads, and eat your greens cooked instead. The reason is that the cruciferous vegetables contain goitrogens that may disrupt the thyroid if consumed in large quantities. Other cruciferous veggies include cabbage, Brussels sprouts, broccoli and cauliflower.
Initial strategies for thyroid hormone replacement included thyroid transplantation, but efficacious pharmacologic strategies soon won favor. Natural thyroid preparations containing T4 and T3, such as desiccated thyroid, thyroid extracts, or thyroglobulin, were the initial pharmacologic agents. Synthetic agents were synthesized later. Early clinical trials demonstrated the efficacy of synthetic and natural agents, but concerns arose regarding consistency of natural thyroid preparations and adverse effects associated with T3-containing preparations (natural or synthetic). With the demonstration of peripheral T4-to-T3 conversion and the availability of the serum TSH radioimmunoassay in the early 1970s, there was a major trend in prescribing preference toward l-thyroxine monotherapy. Apart from conventional medical treatment of hypothyroidism and changing your diet and eating habits, there are also a variety of alternative therapies that could help treat the condition. Physical fitness programs can help cope with the disease. Studies into the effects of yoga on hypothyroidism patients have supported claims of its health benefits, as the practice was found to be useful in the management of disease related symptoms.
Pregnancy . The reason isn’t clear, but sometimes, inflammation of the thyroid occurs after pregnancy. This is called postpartum thyroiditis. Women with this condition usually have a severe increase in thyroid hormone levels followed by a sharp drop in thyroid hormone production. Most women with postpartum thyroiditis will regain their normal thyroid function.
** Medications** - Some medications can contribute to hypothyroidism. Medicines such as lithium, amiodarone, interleukin-2 and interferon alpha can prevent the thyroid gland from producing its hormones normally. These medicines are most likely to affect the thyroid’s functionality in patients who have a genetic susceptibility to autoimmune thyroid disease.
Remember that there is no magic answer, single supplement, or sole dietary change that will miraculously cause you to lose weight. Likewise, medication alone may not be enough to help you feel your best with thyroid disease, whether you have weight to lose or not. Ensuring optimal thyroid function and focusing on diet, movement, and nutritional and lifestyle changes can all help you achieve greater success.
