Try this: Purée raw pumpkin seeds with avocado chunks, cilantro, and a squeeze of lime for a creamy twist on guacamole. Combine pumpkin seeds, canned black beans, shredded carrots, and instant oats in a food processor; pulse until finely chopped and form into burgers; fry until crispy on the outside and cooked through. Or toss pumpkin seeds with melted butter or coconut oil, honey, cinnamon, and cardamom, and toast in the oven at 300°F until browned.
A clinical trial investigating symptoms found that patients receiving l-thyroxine monotherapy, even with a normal TSH, displayed substantial impairment in psychological well-being compared with controls of similar age and sex (3). Because some hypothesized that this phenomenon came about only after adoption of l-thyroxine monotherapy, a study assessed combination therapy with l-thyroxine and l-triiodothyronine. Remarkably, the latter study showed that psychological measures improve in patients receiving combination therapy until serum TSH level is normal (6). In another study comparing l-thyroxine monotherapy versus desiccated thyroid, in which both groups had a normal TSH, many patients preferred desiccated thyroid and lost weight (60). Unfortunately, the solution to this complex problem is not as simple as reverting to combination therapy; the more than a dozen clinical trials on the subject have not shown benefit of superiority and preference for combination therapy, as previously reviewed (1, 3, 70).

According to Dr. Datis Kharrazian, 90% of people with hypothyroidism have Hashimoto’s, an autoimmune hypothyroid condition, whereby the immune system attacks thyroid tissue. Therefore, to cure thyroid disease, or any autoimmune condition, you have to get to the source of the imbalance; focusing on suppression of symptoms with medication is simply barking up the wrong tree.
With the availability of multiple forms of thyroid hormone replacement, early clinical trials were designed to assess efficacy and dose equivalency among natural thyroid (typically desiccated), synthetic l-thyroxine, and/or l-triiodothyronine. These were not designed as superiority trials, their therapeutic goals were the normalization of serum PBI or BMR, and doses were dramatically higher than used today. For example, desiccated thyroid and intravenous l-thyroxine monotherapy normalized BMR, pulse, and body weight in myxedema (29), l-triiodothyronine monotherapy was likewise effective (30), and the potency of l-triiodothyronine exceeded that of l-thyroxine (31).

A discussion on thyroid disease and good health isn’t complete without stressing the importance of physical activity. Lisa Lilienfield, MD, a thyroid disease specialist at the Kaplan Center for Integrative Medicine in McLean, Virginia, and a certified yoga instructor, is a firm believer in the importance of exercise, particularly for clients with a thyroid disorder. “With hypothyroid patients, certainly exercise can help with weight gain, fatigue, and depression. With hyperthyroidism, anxiety and sleep disturbances are so common, and exercise can help regulate both.”
l-Thyroxine monotherapy for athyreotic rats results in a high T4:T3 ratio at doses sufficient to normalize serum TSH levels (8). Yet, the brain, liver, and skeletal muscle tissues of these l-thyroxine–treated animals continue to exhibit markers of hypothyroidism (9), probably because of the inability of l-thyroxine monotherapy to restore tissue levels of T3 (8). This is probably a direct consequence of lower serum T3 levels and the relatively high T4 concentration in these tissues, which inactivates the type 2 iodothyronine deiodinase (D2). In the hypothalamus, loss of D2 is minimal in the presence of T4, which increases sensitivity to T4 levels and explains TSH normalization, despite relatively lower levels of serum T3. Only combination therapy with l-thyroxine plus l-triiodothyronine normalized all thyroid hormone–dependent measures (9), including serum and tissue T3 levels (8). Whether tissue-specific markers of hypothyroidism are restored with l-thyroxine monotherapy in humans remains to be determined, as does the ability of l-thyroxine plus l-triiodothyronine combination therapy to normalize the serum T4:T3 ratio without adverse events. The development of a novel drug delivery system for l-triiodothyronine would facilitate these studies (5).
The tendency to put on weight if you have hypothyroidism can cause people to starve themselves or eat an extremely low-calorie diet. This can cause more harm than good and lead to several other health complications. Instead of fad or crash dieting, learn to eat a healthy balanced meal that provides you with all the necessary nutrients, vitamins and minerals required to function optimally. In addition to this add at least an hour of exercise thrice a week and you can boost your metabolism and reduce symptoms such as fatigue as well.
You must take synthetic thyroxine every day in the morning on an empty stomach. Wait at least 30 minutes before eating or drinking (with the exception of water). Skipping doses can cause your thyroid to go off balance. If you do miss a dose, be sure to take it the next day according to your regular schedule. Don’t double up on your dose by taking two pills at a time, because this can increase your levels by too much.
Seaweed has a high concentration of iodine, an essential nutrient for thyroid function. "Iodine is the precursor for the production of thyroid hormone," Dr. Dodell explains. Seaweed, packaged as nori, wakame, and dulse, can be used in sushi, soups, and salads. Another plus: Seaweed offers nutritional benefits of fiber, calcium, and vitamins A, B, C, E, and K.
Initial strategies for thyroid hormone replacement included thyroid transplantation, but efficacious pharmacologic strategies soon won favor. Natural thyroid preparations containing T4 and T3, such as desiccated thyroid, thyroid extracts, or thyroglobulin, were the initial pharmacologic agents. Synthetic agents were synthesized later. Early clinical trials demonstrated the efficacy of synthetic and natural agents, but concerns arose regarding consistency of natural thyroid preparations and adverse effects associated with T3-containing preparations (natural or synthetic). With the demonstration of peripheral T4-to-T3 conversion and the availability of the serum TSH radioimmunoassay in the early 1970s, there was a major trend in prescribing preference toward l-thyroxine monotherapy. Short of eating a few kelp salads, you probably don't have to worry about getting too much iodine from any other foods. In particular, dairy products are full of this nutrient (and in more manageable amounts), according to a 2012 research in the journal Nutrition Reviews. Part of the reason is because livestock are given iodine supplements and the milking process involves iodine-based cleaners. Plain, low-fat yogurt, or Greek yogurt is a good source—it can make up about 50% of your daily intake of iodine.
The normal values for the serum T4:T3 ratio are seldom discussed in the literature because measurement of serum T3 levels is not a recommended outcome in hypothyroidism (1). In a large study of approximately 3800 healthy individuals (4), the serum free T4:free T3 ratio was around 3, as opposed to a ratio of 4 in more than 1800 patients who had undergone thyroidectomy and were receiving l-thyroxine monotherapy. The corresponding serum free T4:free T3 ratio in patients continuing to receive desiccated thyroid is not well-defined, but the serum total T4:T3 ratio is known to be low (28, 50). In one study, the serum total T4:total T3 was about 40 in patients receiving desiccated thyroid and about 100 in those taking l-thyroxine monotherapy (60). Of course, this is affected by the timing of blood collection in relation to the timing of l-triiodothyronine administration, which is not commonly reported. Other key factors are the well-known poor reproducibility of the serum total T3 assay (61) and the interferences with direct measurement of free T3 (5).
Hyperthyroidism, particularly Graves’ disease, is known to cause bone loss, which is compounded by the vitamin D deficiency commonly found in people with hyperthyroidism. This bone mass can be regained with treatment for hyperthyroidism, and experts suggest that adequate bone-building nutrients, such as vitamin D, are particularly important during and after treatment.13
The association between hypothyroidism and energy expenditure was suspected clinically, and the discovery of lower O2 consumption in myxedema provided an early diagnostic tool (19). The development of a device to assess energy expenditure through measurement of the basal metabolic rate (BMR) in humans proved to be useful for not only diagnosis but also titration of therapy (20). The scale was calibrated so that a normal BMR reference range would be around 0%, whereas athyreotic individuals could have a BMR of about −40% (21). Because of lack of specificity (for example, low BMR in malnutrition), BMR was used in conjunction with the overall clinical impression; a low BMR in the setting of high clinical suspicion would secure a diagnosis and justify treatment (21, 22).

To ensure that you remain as healthy as possible it is important to eat the right variety of foods in the correct proportions. Less common causes of hypothyroidism include congenital (birth) defects (one of the reasons for newborn screening is to check for failure of the pituitary gland to produce enough thyroid stimulating hormone, usually due to a benign pituitary tumor), and pregnancy. Some women can develop hypothyroidism during or immediately following pregnancy, often as a result of developing antibodies against their own thyroid tissue. This is dangerous for both the developing fetus and mother, and can lead to miscarriage, developmental abnormalities, premature delivery and an increased risk of preeclampsia – a potentially dangerous complication in the later stages of pregnancy.
When the hypothalmus decides we need more thyroid hormone in circulation (cold weather or increased activity level for example) it sends a chemical messenger called thyrotropin-releasing hormone (TRH) which goes to the pituitary gland. The pituitary than sends thyroid stimulating hormone (TSH) over to the thyroid. TSH activates the production of a protein called thyroglobulin.
In other words, taking supplements and herbs alone is usually not the solution. Plus, while nutritional supplements can help, one must remember that different people will need to take different types of supplements, take different dosages, etc. Plus the quality of the supplements you take is important, and the truth is, many supplements are of low quality. So just taking a general thyroid support formula is usually not the answer to restoring your health back to normal. Doing so not only can worsen your symptoms, but often times taking this approach will be a complete waste of money. In most cases it takes a combination of different factors to restore one’s health back to normal. This includes eating well, doing a good job of managing stress, getting sufficient sleep, improving the health of the gut, minimizing your exposure to environmental toxins, etc.
Some findings suggest that many people with Hashimoto’s disease (the most common type of hypothyroidism) have lower levels of vitamin D compared to the general population . That’s bad news, since low D is tied to higher levels of thyroid antibodies. “The antibodies activate the immune system to attack the thyroid tissue, which creates inflammation and makes it harder for the thyroid to do its job,” explains Lisa Markley, RDN, co-author of The Essential Thyroid Cookbook.
Another great source of selenium, nuts make a handy snack that you can take anywhere. They also go well in salads or stir-fries. Brazil nuts, macadamia nuts, and hazelnuts are all particularly high in selenium, which helps the thyroid function properly. With Brazil nuts, you only need to eat one or two; with other nuts, a small handful is enough to get your daily nutrients — and be sure to keep an eye on portion size, as nuts are also very high fat.
AGEs cause massive destruction throughout the body and have an affinity for thyroid tissue. Elevated HgA1C (a measure of glycation) is correlated with increased TSH and decreased free T3 & T4 (57). When the blood sugar drops too low (hypoglycemia), it increases stress hormone (cortisol and adrenaline) to boost up blood sugar. Cortisol directly inhibits the enzyme (5’-deiodinase) which converts inactive T4 into active T3.
I don’t know all your symptoms or health challenges, but if you have thyroid issues and if you are losing hair (alopecia areata?), you may want to consider getting tested for celiac disease. It is quite common for people to have celiac disease and thyroid disease. It’s important not to eliminate gluten from your diet before being tested as it would cause a false-negative test result.
*Cassava bears special mention here. You may have heard of it because it is the starchy root vegetable from which tapioca is made, but cassava is also a popular staple food in many Third World countries, where it is eaten boiled, mashed, or ground into flour. Fresh cassava root contains a harmless substance called linamarin, which can turn into hydrocyanic acid (aka cyanide!) when the plant is damaged or eaten. Flaxseeds also contain linamarin. Cyanide is very toxic, so the human body converts it into thiocyanate (which, although it does interfere with thyroid function, is less toxic than cyanide and easier for the body to eliminate).
Alcohol consumption can wreak havoc on both thyroid hormone levels in the body and the ability of the thyroid to produce hormone. Alcohol appears to have a toxic effect on the thyroid gland and suppresses the ability of the body to use thyroid hormone. Ideally, people with hypothyroidism should cut out alcohol completely or drink in careful moderation.
