Almost 5 percent of the U.S. population over the age of 12 has some form of hypothyroidism. (1) Some estimates suggest up to 40 percent of the population suffers from at least some level of underactive thyroid. Women — especially older women — are the most susceptible group for developing hypothyroidism. People who are elderly or who have other existing autoimmune diseases — like type 1 diabetes, rheumatoid arthritis and celiac disease, for example — are also at a higher risk.
Foods that contain some vitamin D include fatty fish, milk, dairy, eggs, and mushrooms. Sunlight also is a potential source, but the amount of vitamin production depends on the season and latitude. If clients have low vitamin D levels, supplemental D3 may be necessary, and the client’s physician should monitor progress to ensure the individual’s levels stay within an appropriate range.
Since most cases of hypothyroidism are permanent and often progressive, it is usually necessary to treat this condition throughout one’s lifetime. Periodic monitoring of TSH levels and clinical status are necessary to ensure that the proper dose is being given, since medication doses may have to be adjusted from time to time. Optimal adjustment of thyroid hormone dosage is critical, since the body is very sensitive to even small changes in thyroid hormone levels.
Thyroid hormones regulate cholesterol synthesis, cholesterol receptors, and the rate of cholesterol degradation. Hypothyroidism increases LDL levels, and increased cholesterol levels have been shown to induce hypothyroidism in animal models. Normalization of thyroid hormone levels has a beneficial effect on cholesterol, which may be worth noting especially for clients who choose not to take prescribed thyroid medications.7
Other causes of hypothyroidism include surgical removal of the thyroid (usually for cancer), radiation therapy of the head and neck, or complications of medical therapies for hyperthyroidism. (Patients with overactive thyroids are often treated with radioactive iodine or anti-thyroid medications that reduce thyroid functioning. These effects can be extensive and permanent, and thyroid supplementation is often required flowing these interventions.) Certain medications can worsen or promote hypothyroidism or interfere with thyroid replacement therapy. One such drug is lithium, used for treating psychiatric conditions such as bipolar disorder.
Think twice before reheating your plastic bowl of takeout soup or keeping that frozen dinner in its original container when you microwave it. Put it on a plate or in a bowl made from ceramics like bone china, stoneware, porcelain or glazed earthenware. Your thyroid is part of your endocrine system, and you can disrupt it by heating food in plastic. The National Institute of Environmental Health Sciences says endocrine disruptors are in many everyday plastic products, including bottles, food, and containers with BPA. Endocrine disruptors work by mimicking naturally occurring hormones in the body, like thyroid hormones.
Hyperthyroidism usually is treated with medications, surgery, or oral radioactive iodine. However, these treatments are imprecise and may cause the thyroid to secrete inadequate amounts of T3 and T4 and function insufficiently after treatment. Seventy percent to 90% of patients with Graves’ or thyroid cancer eventually need treatment for hypothyroidism as a result of treatment.6
Giving appropriate doses of T3 is trickier than appropriately dosing T4. T4 is inactive, so if you give too much there is no immediate, direct tissue effect. T3 is a different story, though, as it is the active thyroid hormone. So if you give too much T3, you can produce hyperthyroid effects directly—a risk, for instance, to people with cardiac disease. 
In areas of the world where there is an iodine deficiency in the diet, severe hypothyroidism occurs in about 5% to 15% of the population. Examples of these areas include Zaire, Ecuador, India, and Chile. Severe iodine deficiency occurs in remote mountain areas such as the Andes and the Himalayas. Since the addition of iodine to table salt and to bread, iodine deficiency is rare in the United States.

Hypothyroidism Diet: One of the main causes of hypothyroidism is inflammation, so following an anti-inflammatory diet is key to improving your thyroid function. Likewise, ensuring your diet is rich in nutrient-dense foods, particularly iodine and selenium, will also help your thyroid produce sufficient levels of thyroid hormones. Some of the best foods to eat for your thyroid: wild-caught fish, coconut oil and ghee, seaweed, probiotic-rich foods like yogurt, sauerkraut and miso, sprouted whole grains and nuts, fiber-rich fruits and vegetables, bone broth, and plenty of good ole’ H20. Essential fatty acids found in fish oil are critical for brain and thyroid function. DHA and EPA omega-3s found in fish oil are associated with a lower risk for thyroid symptoms, including anxiety, depression, high cholesterol, inflammatory bowel disease, arthritis, diabetes, a weakened immune system and heightened autoimmune disease. Omega-3 fish oil supplements can also help balance levels of omega-6s in the diet, which is important for ongoing health.
Physicians hesitated to use l-thyroxine monotherapy over concern that it could result in a relative T3 deficiency, despite growing discontent with potency of natural thyroid products (39) and reduced cost of l-thyroxine, such that the 2 treatments were approximately equivalent (36, 41). The seminal discovery of peripheral T4-to-T3 conversion in athyreotic individuals largely obviated this concern (42). This laid the foundation for the corollary that treatment with l-thyroxine could replace thyroid hormone in such a way that the prohormone pool would be restored and the deiodinases would regulate the pool of active T3. Within a decade there was a major transition toward l-thyroxine monotherapy as first-line therapy (Appendix Table and Figure) (38).

I suspect that there is actually enough iodine in the environment to go around, and that we actually need less than 150 micrograms per day of iodine.  From the above list, you can see that animal foods are much richer in iodine than plant foods—so how do herbivores (animals which eat a plant-based diet, such as rabbits and deer) get enough iodine?  I suspect that there is something about the human diet which interferes with our ability to absorb, utilize, and/or retain iodine, and that this is why we appear to be iodine-deficient compared to other animals.  So, what might the possible culprits be?   Hmmm….
Thyroid scanning is used to determine how active the thyroid is in manufacturing thyroid hormone. This can determine whether inflammation of the thyroid gland (thyroiditis) is present. It can also detect the presence and degree of overactivity of the gland (hyperthyroidism) or, conversely, it can determine the presence and degree of underactivity of the gland (hypothyroidism).
To document that this was a result of trends toward lower doses, an unblinded study tracked well-being according to various doses and found that the highest well-being was achieved at supraoptimal doses, resulting in a suppressed TSH (65). However, a blinded trial did not reproduce this finding (66). In a call to the public, a 1997 British Thyroid Foundation newsletter asked readers to recount personal history of residual hypothyroid symptoms. More than 200 patients responded, 54 of whom specifically mentioned that they did not feel well despite normal serum markers of thyroid function (67, 68). Because of this surge in symptomatic patients, some clinicians advocated titrating dose by symptoms rather than serum TSH, reminiscent of the period before the 1970s (69).

Why does this happen? The immune system mistakenly thinks that the thyroid cells are not a part of the body, so it tries to remove them before they can cause damage and illness. The problem is that this causes widespread inflammation, which can result in many different problems. According to Dr. Datis Kharrazian, 90 percent of people with hypothyroidism have Hashimoto’s that inflames the thyroid gland over time, but this isn’t the only cause of hypothyroidism.


Less common causes of hypothyroidism include congenital (birth) defects (one of the reasons for newborn screening is to check for failure of the pituitary gland to produce enough thyroid stimulating hormone, usually due to a benign pituitary tumor), and pregnancy. Some women can develop hypothyroidism during or immediately following pregnancy, often as a result of developing antibodies against their own thyroid tissue. This is dangerous for both the developing fetus and mother, and can lead to miscarriage, developmental abnormalities, premature delivery and an increased risk of preeclampsia – a potentially dangerous complication in the later stages of pregnancy.
