Also available on the market are combination medications that contain both synthetic T4 and T3 hormones, but such medications aren’t usually recommended. For one thing, most patients see their condition improve with synthetic T4 alone because of the ability of the thyroid to convert these hormones to T3 when needed. Although I’m a fan of getting what you need from real food whenever possible, herbs, supplements, and vitamins can still have a place in the thyroid diet plan. It’s hard to give tips here as many people over-medicate and only feel worse later. Iodine:  Iodine is critical for thyroid hormone production in the body.  A true iodine deficiency will cause hypothyroidism (43).  In western culture we often see subclinical iodine deficiencies which contribute to hypothyroidism (44). I typically don’t recommend high doses of iodine as it could be problematic with individuals with Hashimoto’s – especially with TPO anti-bodies.

Too much thyroid hormone (hypERthyroidism), and your body goes into overdrive; not enough thyroid hormone (hypOthyroidism), and your body slows down.  The most common causes of hypothyroidism worldwide are dietary—protein malnutrition and iodine deficiency.  This is because the two main ingredients needed to make thyroid hormone are tyrosine (an amino acid from dietary protein) and iodine (a naturally-occurring salt).
Could kale, that superstar among superfoods, actually not be quite so awesome? Kale is a mild goitrogen -- in rare cases it prevents the thyroid from getting enough iodine. But kale shouldn't be a problem for you unless you get very little iodine in your diet and you’re eating large amounts of kale. This is also the case for cabbage, broccoli, cauliflower, and Brussels sprouts.  

Kelp? No, but don’t take it in supplement form. Thyroid patients should not have more than an average daily recommended intake of 158 to 175 micrograms of kelp per day, Dr. Nasr says. The concentration of kelp in foods is generally not enough to cause a problem. If you eat kelp once a day, that’s not a problem.”

l-Thyroxine monotherapy for athyreotic rats results in a high T4:T3 ratio at doses sufficient to normalize serum TSH levels (8). Yet, the brain, liver, and skeletal muscle tissues of these l-thyroxine–treated animals continue to exhibit markers of hypothyroidism (9), probably because of the inability of l-thyroxine monotherapy to restore tissue levels of T3 (8). This is probably a direct consequence of lower serum T3 levels and the relatively high T4 concentration in these tissues, which inactivates the type 2 iodothyronine deiodinase (D2). In the hypothalamus, loss of D2 is minimal in the presence of T4, which increases sensitivity to T4 levels and explains TSH normalization, despite relatively lower levels of serum T3. Only combination therapy with l-thyroxine plus l-triiodothyronine normalized all thyroid hormone–dependent measures (9), including serum and tissue T3 levels (8). Whether tissue-specific markers of hypothyroidism are restored with l-thyroxine monotherapy in humans remains to be determined, as does the ability of l-thyroxine plus l-triiodothyronine combination therapy to normalize the serum T4:T3 ratio without adverse events. The development of a novel drug delivery system for l-triiodothyronine would facilitate these studies (5).

Your thyroid needs iodine to work properly and produce enough TH for your body's needs. Don't get enough iodine, and you run the risk of hypothyroidism or a goiter (a thyroid gland that becomes enlarged to make up for the shortage of thyroid hormone). Most Americans have no problem getting enough iodine, since table salt is iodized—but if you're on a low-sodium diet (as an increasing number of Americans are for their heart health) or follow a vegan diet (more on that later), then you may need to up your intake from other sources.

But as a general rule, shellfish like lobster and shrimp are good sources of iodine, she says. In fact, just 3 ounces of shrimp (about 4 or 5 pieces) contains more than 20% of your recommended intake. Bonus: shellfish can also be a good source of zinc, too. Three ounces of Alaskan crab and lobster contain 6.5 and 3.4 milligrams of zinc, respectively.

First things first, you must consider food to be your medicine and get off all processed junk food, sugar (which sends you on a hormonal rollercoaster ride) and gluten. Medications that are used to treat an overactive thyroid (hyperthyroidism) may cause hypothyroidism. These drugs include methimazole (Tapazole) and propylthiouracil (PTU). The psychiatric medication, lithium (Eskalith, Lithobid), is also known to alter thyroid function and cause hypothyroidism. Interestingly, drugs containing a large amount of iodine such as amiodarone (Cordarone), potassium iodide (SSKI, Pima), and Lugol's solution can cause changes in thyroid function, which may result in low blood levels of thyroid hormone.
Whether you take these minerals in a multivitamin or alone, calcium and iron supplements may counteract the medication you take to treat your underactive thyroid. These supplements may affect your ability to absorb levothyroxine, the synthetic thyroid hormone found in medications such as Synthroid and Levothroid, according to the Mayo Clinic. “There’s a very strict way to take thyroid medication,” Blum says. You take it the same way every day, at least one hour before food and never with calcium, iron or other minerals. Blum recommends taking your thyroid medication as soon as you wake up and consuming the mineral supplements with food at dinnertime or before bed.
Short of eating a few kelp salads, you probably don't have to worry about getting too much iodine from any other foods. In particular, dairy products are full of this nutrient (and in more manageable amounts), according to a 2012 research in the journal Nutrition Reviews. Part of the reason is because livestock are given iodine supplements and the milking process involves iodine-based cleaners. Plain, low-fat yogurt, or Greek yogurt is a good source—it can make up about 50% of your daily intake of iodine.

If you have hypothyroidism, you may not have an iodine deficiency per se, but your thyroid is almost certainly struggling in some way to get ahold of the iodine available to it and do what it needs to do with it. If the root cause is left unaddressed, simply increasing iodine is not always useful and at worst can be dangerous depending on how high you’re increasing your supplementation thinking if a little is good, then more will “solve” your problem.
Large predator fish—tuna, swordfish, shark, kingfish, mackerel—often have more mercury than smaller fish, as they’ve lived longer and had more time to accumulate harmful chemicals. Don’t eat more than two to three servings of these fish a week, Blum says. Also, farmed fish like salmon can have higher levels of mercury because they’re often fed the chum of other fish. All fish have a little mercury, so don’t freak out about it. Just don’t order in sushi every weeknight.

From the early 1890s through the mid-1970s, desiccated thyroid was the preferred form of therapy for hypothyroidism (Appendix Table, available at www.annals.org). This preference was reinforced by the unique ability of desiccated thyroid to reproduce a normal serum PBI (33). Concerns about inconsistencies in the potency of these tablets arose (26) after the discovery that some contained anywhere from double to no detectable metabolic activity (39). The shelf-life of desiccated tablets was limited, especially if the tablets were kept in humid conditions (36). There were reports of patients not responding to desiccated thyroid altogether because their tablets contained no active thyroid hormone. It was not until 1985 that the revision of the U.S. Pharmacopeia standard from iodine content to T3/thyroxine (T4) content resulted in stable potency (38), but by then the reputation of natural thyroid products was tarnished (40).
When it comes to thyroid medications, it’s important for RDs to know the medications can interact with common nutritional supplements. Calcium supplements have the potential to interfere with proper absorption of thyroid medications, so patients must consider the timing when taking both. Studies recommend spacing calcium supplements and thyroid medications by at least four hours.21 Coffee and fiber supplements lower the absorption of thyroid medication, so patients should take them one hour apart.22 Dietitians should confirm whether clients have received and are adhering to these guidelines for optimal health.

There are some people who say that there is no scientific evidence linking food to thyroid problems or healing. We have a choice to make about how we want to view things and about what we want to believe. Choice is a powerful tool. Let us never forget that.
