Intravenous immunoglobulins may be used to control SLE with organ involvement, or vasculitis. It is believed that they reduce antibody production or promote the clearance of immune complexes from the body, even though their mechanism of action is not well understood.[87] Unlike immunosuppressives and corticosteroids, IVIGs do not suppress the immune system, so there is less risk of serious infections with these drugs.[88]

The loss of self-tolerance is believed to be due to many hereditary and environmental factors and occurs when autoantigens are damaged, when they link with a foreign antigen, when the structure of a autoantigen is very similar to that of a foreign antigen (molecular mimicry), or when autoreactive T cells are not adequately controlled or are activated by nonspecific antigens. The changes in the appearance of the autoantigen or activation of autoreactive T-cells result in autoantigens being perceived as foreign. Inflammation and destruction of the tissues bearing the antigen occur because of the production of autoantibodies by B cells or the cytotoxicity of autoreactive T cells, which attack the autoantigens.
This diet is not intended for weight loss (although that can be a side effect). The anti-inflammatory diet is intended to provide steady energy, plenty of vitamins and minerals, and the essential fatty acids needed to maintain optimum health. You could look at this more like an eating plan for life as opposed to a diet per se. It is based on the general concept that eating to avoid inflammation promotes better health and can ward off diseases. According to Dr. Andrew Weil, the Harvard trained natural and preventative medicine physician (as seen on Oprah, and the Dr. Oz show,) there is clear evidence to support that inflammation can be very damaging to the body and he therefore openly supports the Anti-Inflammatory Diet. “We all know inflammation on the surface of the body as local redness, heat, swelling and pain. It is the cornerstone of the body’s healing response, bringing more nourishment and more immune activity to a site of injury or infection. But when inflammation persists or serves no purpose, it damages the body and causes illness. Stress, lack of exercise, genetic predisposition, and exposure to toxins (like secondhand tobacco smoke) can all contribute to such chronic inflammation, but dietary choices play a big role as well.” Both he and Barry Sears, MD, the author of the well-known Zone Diet both agree that this diet can have significant positive results on many diseases. Here are the basics of the anti-inflammatory diet (all versions vary, but this is the general proposal for all:
An antibody, produced by B cells in response to an altered autoantigen on one type of the body’s own cells, that attacks and destroys these cells. Autoantibodies are the basis for autoimmune diseases such as rheumatoid arthritis and diabetes mellitus. Several theories exist about why autoantibodies are formed. The most common theory proposes that AAbs develop as the result of a combination of hereditary and environmental risk factors that cause an autoantigen to be falsely recognized as alien by B cells; as a result, antibodies are produced for its destruction.
Research has demonstrated evidence that a key enzyme's failure to dispose of dying cells may contribute the development of systemic lupus erythematosus. The enzyme, DNase1, normally eliminates what is called "garbage DNA" and other cellular debris by chopping them into tiny fragments for easier disposal. Researchers turned off the DNase1 gene in mice. The mice appeared healthy at birth, but after six to eight months, the majority of mice without DNase1 showed signs of systemic lupus erythematosus. Thus, a genetic mutation in a gene that could disrupt the body's cellular waste disposal may be involved in the initiation of systemic lupus erythematosus.

Cells are the basic building blocks of all living things. The human body is composed of trillions of cells. They provide structure for the body, take in nutrients from food, convert those nutrients into energy, and carry out specialized functions. Cells also contain the body’s hereditary material and can make copies of themselves. When someone has lupus, the immune system can’t tell the difference between the body’s healthy cells and bacteria and viruses, so the antibodies attack the body’s healthy cells.
Soy products. Soy products are high in a type of estrogen called phytoestrogen, and estrogen is known to be a risk factor for lupus. In animal studies, researchers noted that a diet high in soy seemed to make lupus symptoms worse. Although there is no definitive evidence that soy products cause lupus symptoms, you should be cautious about including large amounts of soy in your diet.
Osteoarthritis is the most common form of arthritis, affecting millions of people around the world. Often called wear-and-tear arthritis, osteoarthritis occurs when the protective cartilage on the ends of your bones wears down over time. While osteoarthritis can damage any joint in your body, the disorder most commonly affects joints in your hands, neck, lower back, knees and hips. Osteoarthritis gradually worsens with time, and no cure exists. But osteoarthritis treatments can slow the progression of the disease, relieve pain and improve joint function.
A member of a group of enzymes that can break down proteins, such as collagen, that are normally found in the spaces between cells in tissues (i.e., extracellular matrix proteins). Because these enzymes need zinc or calcium atoms to work properly, they are called metalloproteinases. Matrix metalloproteinases are involved in wound healing, angiogenesis, and tumor cell metastasis.

If cyclophosphamide doesn’t work for patients with lupus nephritis, they now have the option of trying another drug, and more options are being tested in clinical trials, says Dr. Caricchio. For example, doctors may use CellCept (mycophenolate mofetil), Imuran (azathioprine), or Restasis (cyclosporine). Prograf (tacrolimus) may also be an effective option for lupus nephritis, according to study data appearing in the January 2016 edition of Autoimmunity Reviews. The European League Against Rheumatism (EULAR) vaccination recommendations for rheumatic diseases, including lupus, advocate baseline assessment and delivery of nonlive vaccines during stable disease. [150] Particularly important is immunization against encapsulated organisms, such as meningococcal vaccine, pneumococcal vaccine, and routine Haemophilus influenzae childhood vaccination. Annual influenza vaccine is also encouraged.

In its simplest definition, the CBC is used to measure red and white blood cell count, the total amount of hemoglobin in the blood, hematocrit (the amount of blood composed of red blood cells), and mean corpuscular volume (the size of red blood cells). The CBC can also count additional blood cell types like neutrophils, eosinophils, basophils, lymphocytes, monocytes, and platelets.
Antimalarials are another type of drug commonly used to treat lupus. These drugs prevent and treat malaria, but doctors have found that they also are useful for lupus. A common antimalarial used to treat lupus is hydroxychloroquine. It may be used alone or in combination with other drugs and generally is used to treat fatigue, joint pain, skin rashes, and inflammation of the lungs. Clinical studies have found that continuous treatment with antimalarials may prevent flares from recurring.

The Accelerating Medicines Partnership (AMP) is a bold new venture between the NIH, 10 biopharmaceutical companies and several non-profit organizations to transform the current model for developing new diagnostics and treatments by jointly identifying and validating promising biological targets of disease. The ultimate goal is to increase the number of new diagnostics and therapies for patients and reduce the time and cost of developing them.
Limited evidence suggests that supplementation may be clinically beneficial in SLE patients with low levels of vitamin D. In Mediterranean patients,  female patients who were not receiving supplemental vitamin D showed more fatigue and received more oral corticosteroids than those with normal levels of vitamin D. [109] In Australian patients, an increase in serum vitamin D levels was associated with reduced disease activity over time. [152]
Inflammation associated with lupus can cause stiffness, swelling, pain, and warmth of the joints, most commonly in the fingers, hands, elbows, ankles, and toes. (8) Most people with lupus will experience joint inflammation at some point, says Caricchio. For many people, joint pain is one of the first symptoms of the disease that they’ll notice and report.
In addition to the 11 criteria, other tests can be helpful in evaluating people with SLE to determine the severity of organ involvement. These include routine testing of the blood to detect inflammation (for example, the erythrocyte sedimentation rate, or ESR, and the C-reactive protein, or CRP), blood-chemistry testing, direct analysis of internal body fluids, and tissue biopsies. Abnormalities in body fluids (joint or cerebrospinal fluid) and tissue samples (kidney biopsy, skin biopsy, and nerve biopsy) can further support the diagnosis of SLE. The appropriate testing procedures are selected for the patient individually by the doctor.
Some people find that excluding gluten from their diet gives them more energy. Don’t be tempted to start excluding a lot of foods from your diet; this could lead to serious deficiencies. Avoiding gluten or dairy products will not necessarily prevent flares; food “triggers” vary greatly from person to person.  If you feel that you have problems processing certain foods, talk to your GP and ask for a referral to either a dietician or an allergy specialist within the NHS. There are commercial allergy tests available, but these are not always accurate and could cost you a lot of money but bring you no lasting benefit.

It’s been found that low levels of vitamin D might be associated with increased risk of autoimmune conditions and other chronic diseases, according to a report published in the International Journal of Rheumatology. (8) If you don’t spend much time outdoors, especially during the winter, talk to your doctor about taking a supplement to prevent vitamin D deficiency.


Disease-modifying antirheumatic drugs (DMARDs). DMARDs do more than just treat the symptoms of lupus. Research has shown that they can modify the course of the disease, prevent progression and slow joint damage. DMARDs are often used with NSAIDs. Hydroxychloriquine commonly is prescribed for people with lupus. It can cause vision changes in some people, so it is important to have regular vision examinations. Hydroxychloriquine is effective in preventing flares.

We acknowledge as a limitation that certainty of the evidence was not as high as desirable for most recommendations and probably biased by few randomised clinical trials. Although regional information was published on several topics1 4 10 11 23 24 31–49 we recognise that these guidelines should be updated as research-based changes in our understanding of SLE emerge. Regardless, the publication of these guidelines must be followed by health system engagement and implementation by specialists, major steps towards improvement of lupus treatment in Latin America and low/middle-income countries.


Drug-induced lupus erythematosus (DIL) Some drugs can cause lupus, resulting in symptoms such as rash, arthritis, hair loss, and fever. “Once medications are discontinued, the symptoms go away,” says Roberto Caricchio, MD, the interim section chief of rheumatology at Temple University Hospital in Philadelphia and the director of the Temple Lupus Clinic at the Lewis Katz School of Medicine.

The cause of Lupus is unknown, as well as what drives its diverse presentation. We know that multiple factors are required, including: the “right” genetic makeup, environmental exposures, and organ specific characteristics. People with lupus may also have an impaired process for clearing old and damaged cells from the body, which in turn provides continuous stimuli to the immune system and leads to abnormal immune response.

A clinical trial is a prospective biomedical or behavioral research study of human subjects that is designed to answer specific questions about biomedical or behavioral interventions (drugs, treatments, devices, or new ways of using known drugs, treatments, or devices). Clinical trials are used to determine whether new biomedical or behavioral interventions are safe, efficacious, and effective.


In more severe cases, medications that modulate the immune system (primarily corticosteroids and immunosuppressants) are used to control the disease and prevent recurrence of symptoms (known as flares). Depending on the dosage, people who require steroids may develop Cushing's syndrome, symptoms of which may include obesity, puffy round face, diabetes mellitus, increased appetite, difficulty sleeping and osteoporosis. These may subside if and when the large initial dosage is reduced, but long-term use of even low doses can cause elevated blood pressure and cataracts.
A complex of genes on chromosome 6 that code for the antigens that determine tissue and blood compatibility. In humans, histocompatibility antigens are called human leukocyte antigens (HLA) because they were originally discovered in large numbers on lymphocytes. There are thousands of combinations of HLA antigens. Class I MHC antigens (HLA-A, HLA-B, and HLA-C) are found on all nucleated cells and platelets. Class II antigens (HLA-DR, HLA-DQ, and HLA-DP) are found on lymphocytes and antigen processing cells and are important in the specific immune response. In tissue and organ transplantation, the extent to which the HLA or “tissue type” of the donor and recipient match is a major determinant of the success of the transplant.
Why the test is used: Abnormalities in blood cell counts, including white blood cells and red blood cells, may occur in people with lupus. This may be related to the lupus, lupus treatments, or infection. For example, leukopenia, a decrease in the number of white blood cells, is found in about 50% of people with lupus. Thrombocytopenia, or a low platelet count, occurs in about 50% of people with lupus, as well. Doctors can use this test to monitor these potentially serious problems.
The ACR recommends ANA testing in patients who have two or more unexplained signs or symptoms listed in Table 2.2,20,21 [Reference2—Evidence level C, consensus/expert guidelines] Because of the high rate of false positive ANA titers, testing for systemic lupus erythematosus with an ANA titer or other autoantibody test is not indicated in patients with isolated myalgias or arthralgias in the absence of these specific clinical signs.45 Under most circumstances, a persistently negative ANA titer (less than 1:40) can be assumed to rule out systemic lupus erythematosus.41
The body’s tolerance of the antigens present on its own cells, i.e., autoantigens or self-antigens. It is theorized that autoreactive T lymphocytes are destroyed in the thymus by negative selection or in peripheral blood. Autoreactive T cells that escape destruction in the thymus may become tolerant because they are exposed to thousands of autoantigens as they circulate in the blood.
ANAs are proteins made by the body that can attach to DNA and other substances inside cells. But just because they are present in the body doesn’t necessarily mean they will attack these substances. These antibodies are found in at least 5% of the general population, so there are "many more people walking around with ANAs who are perfectly healthy or have some illness that has nothing to do with lupus," adds Dr. Belmont.
Genome-wide association studies (GWAS) revealed regions of linkage that were found on most chromosomes.[52] These studies are useful in identifying the genes that may be responsible for complex diseases such as SLE. Candidate gene loci implicated with SLE include multiple alleles from the HLA region, Fc-gamma receptor, and complement component system.[50] However, association does not prove that a specific form of a gene is responsible for the disease, as there may be other polymorphisms in the region that have a greater association effect.[50] However, because the biological role of most genes are not completely understood, it can be difficult to attribute phenotypic traits to certain genetic polymorphisms. Since SLE is associated with so many genetic regions, it is likely an oligogenic trait, meaning that there are several genes that control susceptibility to the disease.[50] Further complicating our understanding is the association of certain linkages with various ethnic groups.[50]
Other diseases and conditions that can accompany lupus include fibromyalgia, coronary heart disease, nonbacterial valvular heart disease, pancreatitis, esophagus disease with difficulty swallowing (dysphagia), swollen lymph nodes (lymphadenopathy), liver disease (lupoid hepatitis), infections, and a tendency to spontaneous blood clotting and thrombosis.

Next, Ms. Everett reviewed some of the key foods that are important for your diet. She emphasized that balance is essential – that is, to not eat too much of one thing and not enough of another. Different foods have different nutritional components. Included in the important foods that Ms. Everett highlighted were a variety of fruits and vegetables; foods low in calories and saturated fats; and foods high in antioxidants, fiber, calcium, vitamin D, and Omega 3 fatty acids.


When the kidneys or central nervous systems are affected immunosuppressive drugs such as cyclophosphamide (Cytoxan) and mycophenolate mofetil (CellCept) may be used. These drugs restrain the overactive immune system by blocking production of immune cells. Side effects may include nausea, vomiting, hair loss, bladder problems, decreased fertility, and increased risk of cancer and infection. The risks increase with the length of treatment.

While there is no specific lupus diet, scientists have found that low-dose diet supplementation with omega-3 fish oils could help patients with lupus by decreasing inflammation and disease activity and possibly decreasing heart-disease risk. It is generally recommended that patients with lupus eat a balanced diet that includes plant-based foods and lean sources of protein.
