In a study published in 2015, patients with SLE were referred for nutrition counseling with a registered dietician (RD), and 41 of 71 referrals participated in the sessions.8 At the end of the 6-month period, the patients who received nutrition counseling were more likely to have lost weight; decreased their intake of foods high in fat, sodium, and calories; and increased their consumption of fruits, vegetables, fiber, and fish.
Subacute cutaneous: The skin symptoms of subacute cutaneous lupus are usually mild. People with this condition, which is also its own form of lupus, present with reddish-purple plaques, which are firm and raised, flattened skin lesions. These plaques can be found alone or in groups and range in size from 5 mm to 20 mm, usually appearing on the trunk, including the upper chest and back. About 10 percent of people with SLE have subacute cutaneous lupus. Certain drugs may also cause subacute cutaneous lupus.
The removal of plasma from a patient (usually to treat an immmunologically mediated illness such as thrombotic thrombocytopenic purpura or myasthenia gravis) and its replacement with normal plasma. Plasma exchange therapy can also be used to replace excessively viscous plasma in patients with Waldenström macroglobulinemia. Pathological antibodies, immune complexes, and protein-bound toxins are removed from the plasma by plasma exchange. Immunoglobulin infusions are an alternative to plasma exchange when treating some immunological illnesses, including Guillain-Barré syndrome and chronic inflammatory demyelinating polyneuropathy.
Other drugs used to treat lupus include the antimalarial drug hydroxychloroquine, which modulates the immune system, and belimumab, a targeted drug that is a biologic (meaning it’s made from natural sources). Some chemotherapy drugs and anti-rejection drugs may be used, too, to treat patients with lupus nephritis or other organ problems, says Caricchio.
In SLE patients with serious brain (lupus cerebritis) or kidney disease (lupus nephritis), plasmapheresis is sometimes used to remove antibodies and other immune substances from the blood to suppress immunity. Plasmapheresis is a process of removing blood and passing the blood through a filtering machine, then returning the blood to the body with its antibodies removed. Rarely, people with SLE can develop seriously low platelet levels, thereby increasing the risk of excessive and spontaneous bleeding. Since the spleen is believed to be the major site of platelet destruction, surgical removal of the spleen is sometimes performed to improve platelet levels. Other treatments have included plasmapheresis and the use of male hormones.
Toxic molds (mycotoxins) and heavy metals such as mercury are the two main toxins I see in those with autoimmune conditions. Mycotoxins are highly toxic substances produced by toxic molds. Only about 25% of the population carries the genes to be susceptible to the effects of mycotoxins.3 Conventional environmental mold testing only tests for levels of mold spores and does not test for mycotoxins. I use a urine mycotoxin test in my clinic to determine if someone has been exposed to toxic molds.
The NIH: National Institute of Arthritis and Musculoskeletal and Skin Diseases (2014) suggests that the symptoms may vary dependent on the type of lupus and the person. Symptoms tend to ‘come and go’, ‘flare’ from mild to severe intensity, and new symptoms of lupus can arise at any stage (NIH, 2014). Better Health Channel (n. d.) state that lupus may even become life-threatening, for example, should it damage major organs such as the kidneys or brain.
Flare-ups of lupus can cause acute inflammation and damage to various body tissues and can affect the joints, skin, kidneys, heart, lungs, blood vessels, and brain. Some of the most common symptoms are painful or swollen joints, unexplained fever, kidney problems and extreme fatigue. A characteristic red skin rash – called a “malar” or “butterfly” rash because it roughly mimics the insect’s shape – may appear across the nose and cheeks. Rashes may also occur on the face and ears, upper arms, shoulders, chest, and hands. Because many lupus patients are sensitive to sunlight, skin rashes often develop or worsen after sun exposure.
We encourage you to reach out to friends, family, and join support groups to share your feelings and fears. Also, remember to be your own best advocate in your journey with lupus, take great notes, and bring a support person with you to each visit to help remind you of the doctor’s advice and information. The monoclonal antibody belimumab (Benlysta), a B-lymphocyte stimulator–specific inhibitor, has been found to reduce disease activity and possibly decrease the number of severe flares and steroid use in patients with SLE when used in combination with standard therapy. In March, 2011, the US Food and Drug Administration (FDA) approved the use of belimumab in combination with standard therapies (including steroids, nonbiologic DMARDS [eg, hydroxychloroquine, azathioprine, methotrexate]) to treat active autoantibody-positive SLE. In July 2017, a subcutaneous (SC) formulation was approved that allows patients to self-administer a once-weekly dose.
I tend to stay away from garlic, never used alfalfa. I know most restaurants will use garlic but when cooking at home, I leave it out and find other seasonings that make food taste good as well. I am more plant-based and cook in more than eating out to keep a better feel for what I’m putting in my body. I do still enjoy a glass of wine once a week or so. I initially did a food elimination phase and that helped me figure out what works for my body. Its been a couple of years and I am actually about to do another one since converting over from vegetarian to plant-based means a few different food options and of course our bodies are always changing their minds about how they want to respond to things.
There are over 200 disorders that impact connective tissue. Some, like cellulitis, are the result of an infection. Injuries can cause connective tissue disorders, such as scars. Others, such as Ehlers-Danlos syndrome, Marfan syndrome, and osteogenesis imperfecta, are genetic. Still others, like scleroderma, have no known cause. Each disorder has its own symptoms and needs different treatment.
Photosensitive systemic lupus erythematosus (SLE) rashes typically occur on the face or extremities, which are sun-exposed regions. Although the interphalangeal spaces are affected, the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) and distal interphalangeal (DIP) joints are spared. “My message to patients is that we can do an excellent job of managing the condition compared to 20 years ago,” says Roberto Caricchio, MD, the interim section chief of rheumatology at Temple University Hospital in Philadelphia and the director of the Temple Lupus Clinic at the Lewis Katz School of Medicine. With that said, people should never underestimate the serious effects lupus can have, he adds, which is why working with your doctor to manage the condition is so important.
