The body’s tolerance of the antigens present on its own cells, i.e., autoantigens or self-antigens. It is theorized that autoreactive T lymphocytes are destroyed in the thymus by negative selection or in peripheral blood. Autoreactive T cells that escape destruction in the thymus may become tolerant because they are exposed to thousands of autoantigens as they circulate in the blood.

Systemic lupus erythematosus, the most common form of the condition, can potentially involve any major organ system of the body, says Neil Kramer, MD, co-medical director at the Institute for Rheumatic and Autoimmune Diseases at Overlook Medical Center in Summit, New Jersey. “Therefore, the first signs and symptoms vary from patient to patient.”

Lupus, also known as Systemic Lupus Erythematosus or SLE, is a complex disease that can be difficult to diagnose. It affects many areas of body including the joints, skin and kidneys. More than 200,000 people in the U.S. are diagnosed with lupus each year. Like other autoimmune diseases, in lupus, cells essentially make the bad decision to attack the body’s own cells.
The ACR Quality of Care statement  recommends annual cardiovascular disease risk assessment; some researchers suggest that the cardiovascular risk for SLE is similar to that for diabetes mellitus. The 10-year coronary event rate is 13-15% in patients with active SLE, which is comparable to the 10-year event rate of 18.8% in patients with known coronary artery disease.  African American patients with SLE may be particularly vulnerable to premature cardiovascular disease and related death. 
For instance, a dermatologist for cutaneous lupus (skin disease), a cardiologist for heart disease, a nephrologist for kidney disease, a neurologist for brain and nervous system disease, or a gastroenterologist for gastrointestinal tract disease. A woman with lupus who is considering a pregnancy needs an obstetrician who specializes in high-risk pregnancies.
