We conducted a systematic evidence-based review of the published literature on systemic lupus erythematosus. After searching several evidence-based databases (Table 1), we reviewed the MEDLINE database using the PubMed search engine. Search terms included “lupus not discoid not review not case” and “lupus and treatment and mortality,” with the following limits: 1996 to present, abstract available, human, and English language. One author reviewed qualifying studies for relevance and method.
Certain people may need to follow a slightly different diet. For example, pregnant women need to avoid eating certain foods; people with lupus nephritis (lupus affecting the kidneys) need to follow advice from their hospital dietician; and dietary advice for people over 60 and for children of various ages may also be different. Lupus is a chronic autoimmune disease that causes the immune system to attack healthy tissue resulting in inflammation, particularly of the skin, joints, kidneys, heart, lungs, blood vessels and brain. It develops most commonly in women between the ages of 15-45, and occurs more often in African-American, Hispanics, Native Americans and Asians. Men can get lupus too. Lupus is not infectious or cancerous. People with lupus may have many different symptoms affecting various parts of the body. Some of the most common symptoms are extreme fatigue, painful or swollen joints (arthritis), unexplained fevers, skin rashes and kidney problems. Lupus is characterized by “flares” or periods of illness and remission. Warning signs of a flare can be increased fatigue, pain, rash, fever, abdominal discomfort, headache or dizziness. Learning how to recognize these signs can help people maintain better health and reduce or ward off a flare. Currently, there is no cure for lupus but it can be managed effectively with drugs, and most people with lupus lead an active, healthy life.
Studies from around the world have documented a higher prevalence of vitamin D insufficiency and deficiency in patients with SLE, compared with the general population, especially in conjunction with obesity. [108, 152, 153, 154, 155, 110] Studies from Australia,  France,  the Mediterranean region,  and Taiwan  —but not from Mexico  —have shown an association between serum vitamin D levels and SLE disease activity.
No diet-based treatment of SLE has been proven effective. Patients with SLE should be reminded that activity may need to be modified as tolerated. Specifically, stress and physical illness may precipitate SLE flares. Additionally, persons with SLE should wear sunscreen and protective clothing or avoid sun exposure to limit photosensitive rash or disease flares.
Most autoimmune diseases affect one specific system. For example, Rheumatoid Arthritis involves the joints, and Multiple Sclerosis affects the brain and spinal cord. Lupus, on the other hand, affects more than one system simultaneously. No matter what organ or system is being attacked, all autoimmune diseases are similar in that they are an immune response caused by systemic inflammation that leads your body to attack itself.
The panel concluded that both MMF plus high-dose GCs (prednisone 1–2 mg/kg/day, maximum 60 mg/day) and CYC plus high-dose GCs are associated with significant benefits in comparison to GCs alone. No significant differences between these two alternatives were noted. The panel pointed that differential pharmacokinetic effects of MMF in cLN may exist, which could require dosing increase.30 Risk of reduction of ovarian reserve and sperm abnormalities should be considered in patients with cLN treated with CYC.
Consuming foods in their natural, whole form limits your exposure to synthetic additives, toxins or pesticides. These chemicals are very commonly found in packaged products and non-organic foods (even many veggies and fruit!). Because those with lupus already have weakened immune systems, reducing exposure to synthetic hormones, chemicals, medications and heavy metals is usually crucial for recovery.
Joints that are red, warm, tender, and swollen may signal lupus. Aching and stiffness alone aren’t enough; the joints have to be affected by arthritis and these other "cardinal signs of inflammation," says Michael Belmont, MD, director of the lupus clinic at Bellevue Hospital and medical director at the New York University Hospital for Joint Diseases in New York City.
The panel judged the effect of extended AC as a large benefit, reducing VTD with increase in bleeding risk as a moderate harm. For the comparisons of different AC intensities, the panel decided to use the evidence from observational studies because it judged that it probably better reflects reality given that the randomised controlled trials (RCT) are severely flawed (indirectness of intervention as most patients did not reach the INR >3 goal). They judged the reduction in VTD as a large benefit and the bleeding increase as a large harm. Hence, the panel considered that the balance could favour the intervention only when the risk of VTD recurrence is particularly high.
At least half of people with lupus experience fatigue. (4) Fatigue may be brought on by the disease itself or from associated depression, anxiety, lack of exercise, and problems with sleep. ( 5) Because people with lupus need to avoid sun exposure, they may have low levels of vitamin D, which can contribute to fatigue. Lupus treatments may also play a role.
A mononuclear phagocytic white blood cell derived from myeloid stem cells. Monocytes circulate in the bloodstream for about 24 hr and then move into tissues, at which point they mature into macrophages, which are long lived. Monocytes and macrophages are one of the first lines of defense in the inflammatory process. This network of fixed and mobile phagocytes that engulf foreign antigens and cell debris previously was called the reticuloendothelial system and is now referred to as the mononuclear phagocyte system (MPS).
On my first (and last) visit to the rheumatologist I asked what I could do to support my health or to avoid a worsening my lupus symptoms. She casually responded "Come back when you're worse and I'll put you on steroids". Straining to get some kind of supportive information I mustered up a question about diet and if there were foods I should eat or avoid. Her response was, "continue to eat whatever you want, it won't make a difference".
A diet high in omega-3 fatty acids may help to mitigate inflammation. Although omega-3s have not been adequately studied in lupus, studies of the general population suggest that these essential fatty acids may also boost mood and improve cardiovascular health. Fish, nuts, and flax are excellent sources of omega-3s and can be easily incorporated into everyday meals. Try to avoid saturated fats, such as those in beef and fried snack foods, since these fats are known to increase the risk of cardiovascular disease and may actually stimulate the immune system.
Chronic diseases are noncommunicable illnesses that are prolonged in duration, do not resolve spontaneously, and are rarely cured completely. Although chronic diseases are more common among older adults, they affect people of all ages and are now recognized as a leading health concern of the nation. Growing evidence indicates that a comprehensive approach to prevention can save tremendous costs and needless suffering.
Chronic fatigue syndrome (CFS) is a disorder that causes extreme fatigue. This fatigue is not the kind of tired feeling that goes away after you rest. Instead, it lasts a long time and limits your ability to do ordinary daily activities. The main symptom of CFS is severe fatigue that lasts for 6 months or more. In recent years, mycophenolate mofetil (CellCept) has been used as an effective medication for lupus, particularly when it is associated with kidney disease. CellCept has been helpful in reversing active lupus kidney disease (lupus renal disease) and in maintaining remission after it is established. Its lower side-effect profile has advantage over traditional immune-suppression medications.
While the onset and persistence of SLE can show disparities between genders, socioeconomic status also plays a major role. Women with SLE and of lower socioeconomic status have been shown to have higher depression scores, higher body mass index, and more restricted access to medical care than women of higher socioeconomic statuses with the illness. People with SLE had more self-reported anxiety and depression scores if they were from a lower socioeconomic status.

There have been several diet studies using omega-3 fatty acids in people who have lupus. A 2012 study looked at the eating habits of 114 SLE patients. They found that those who had a diet low in omega-3 fatty acids had worse lupus disease activity as well as higher levels of cholesterol and atherosclerosis (which can cause heart attacks and strokes). Therefore, it is important for people who have lupus to supplement their diet with foods rich in omega-3 fatty acids, olive oil, or supplements containing these oils. Not only may this possibly improve lupus disease activity, but it may also improve cholesterol levels, which could help to decrease the risk of getting heart attacks, strokes and blood clots.
Raw veggies promote an alkaline environment in the body which can help keep inflammation levels lower. They also supply antioxidants, prebiotics, dietary fiber, and many essential vitamins and minerals. Whether eaten raw or cooked, some of the best choices include leafy greens, garlic, onions, asparagus, artichoke, bell peppers, beets, mushrooms and avocado. These help supply nutrients like the vitamin C, selenium, magnesium and potassium you need. Aim for variety and a minimum of four to five servings per day.
Along with nutritional deficiencies, steroid medications can cause significant weight gain and increased cholesterol, blood glucose, and triglycerides, further underscoring the need for patients with SLE who are taking these agents to follow a healthy diet to counter the effects.6 There are also specific things that individuals with SLE should avoid, including alfalfa sprouts and garlic, which can stimulate an already overactive immune system.7
Limited evidence suggests that supplementation may be clinically beneficial in SLE patients with low levels of vitamin D. In Mediterranean patients, female patients who were not receiving supplemental vitamin D showed more fatigue and received more oral corticosteroids than those with normal levels of vitamin D.  In Australian patients, an increase in serum vitamin D levels was associated with reduced disease activity over time. 
Many women with lupus ask "it safe for me to get pregnant?" or "can I have a healthy pregnancy?" Pregnancy is possible in most patients with lupus, but complications are frequent. Anyone with lupus should be considered to have a high risk for health complications during pregnancy. When a woman with lupus becomes pregnant or is planning to become pregnant, she should get a referral for appropriate, specialized care. Lupus patients who are positive for aPL are at high risk of miscarriage, while patients with anti-Ro/SSA and anti-La/SSB antibodies are at risk for delivering a child with neonatal lupus.
Lupus is a chronic autoimmune condition in which the immune system attacks the body’s own healthy tissue and organs. Depending on the specific patient, lupus can cause high levels of persistent inflammation that can negatively affect various parts of the body. Antinuclear Antibody Test (ANA): A positive ANA test for the presence of these antibodies, which are produced by your immune system, indicates a stimulated immune system. While most people with lupus have a positive ANA test, most people with a positive ANA test do not have lupus. If you have a positive ANA test, more specific antibody testing will most likely be advised.
No single finding qualifies an individual as having SLE. Instead, the American College of Rheumatology (ACR) has devised certain classification criteria, and four or more of these criteria must be present for a classification of lupus. [The term “classification” is not synonymous with “diagnosis.” “Classification” means that reasonable certainty exists for the diagnosis of lupus for research purposes.] Although, these criteria are currently being updated, they are believed to be about 90% effective. The ACR criteria include malar rash; discoid rash; photosensitivity (development of a rash after sun exposure); oral or nasal ulcers; arthritis of multiple joints; serositis: (inflammation of the lining around the lungs or heart); kidney disease indicated by protein or casts in the urine; neurological disorders such as seizures and psychosis; and blood disorders such as hemolytic anemia, leukopenia, and lymphopenia. Other signs that are common but not included in the classification criteria are hair loss or breaking, especially around the forehead, and Raynaud’s Phenomenon, a two- or three-color change of the fingertips upon cold exposure.
Another common comorbidity with SLE is osteoporosis; researchers have found an increased risk of fracture and bone loss in SLE. Experts attribute this to several factors, including glucocorticoid medications that can lead to bone loss, inactivity due to symptoms such as pain and fatigue, and possibly the disease activity itself. In addition, women comprise approximately 90% of people with SLE, adding to their generally elevated osteoporosis risk.5
Systemic lupus erythematosus is a multisystem inflammatory disease that is often difficult to diagnose. Before the diagnosis can be established, four of 11 clinical and laboratory criteria must be met. Antinuclear antibody titer is the primary laboratory test used to diagnose systemic lupus erythematosus. Because of the low prevalence of the disease in primary care populations, the antinuclear antibody titer has a low predictive value in patients without typical clinical symptoms. Therefore, as specified by the American College of Rheumatology, this titer should be obtained only in patients with unexplained involvement of two or more organ systems. Pa tients with an antinuclear antibody titer of 1:40 and characteristic multiorgan system involvement can be diagnosed with systemic lupus erythematosus without additional testing; however, patients with an antibody titer of 1:40 who fail to meet full clinical criteria should undergo additional testing, including tests for antibody to doublestranded DNA antigen and antibody to Sm nuclear antigen. While an antinuclear antibody titer of less than 1:40 usually rules out systemic lupus erythematosus, patients with persistent, characteristic multisystem involvement may be evaluated for possible antinuclear antibody–negative disease.

The clinical manifestations of systemic lupus erythematosus are fundamentally the same in children and adults.15 In two descriptive studies25,26 of children with the disease, the most frequent manifestations were fever, rash, arthritis, alopecia, and renal involvement. Compared with adults, children have a higher incidence of malar rash, anemia, leukocytopenia,27 and severe manifestations such as neurologic or renal involvement.28
Corticosteroids and immune suppressants: Patients with serious or life-threatening problems such as kidney inflammation, lung or heart involvement, and central nervous system symptoms need more “aggressive” (stronger) treatment. This may include high-dose corticosteroids such as prednisone (Deltasone and others) and drugs that suppress the immune system. Immune suppressants include azathioprine (Imuran), cyclophosphamide (Cytoxan), and cyclosporine (Neoral, Sandimmune). Recently mycophenolate mofetil has been used to treat severe kidney disease in lupus – referred to as lupus nephritis.
