"Keeping my weight under control has been a battle. I have tried diets. I know that being overweight increases joint stress and stress on my heart, both of which can be affected by lupus," says LaPlant. Some of the medications that people take for lupus can make it difficult to maintain a healthy weight. Prednisone, one of the most common medications used to treat lupus flares, can increase your appetite and lead to significant weight gain. Regular, low-impact exercise can help offset weight gain and also improve your health in general.

People with lupus are at great risk of contracting kidney disease. Kidney failure occurs in a minority of patients with lupus nephritis, despite advances in therapy. These patients must undergo dialysis. About one-third of patients who start dialysis during an acute lupus flare will be able to discontinue it within the first year. The remaining two-thirds, and those suffering gradual deterioration of kidney function over several years will require either continual dialysis for life or a kidney transplant.
One common early symptom that can be indicative of lupus is a photosensitive rash, meaning a rash that develops in response to sun exposure, particularly on the face and upper arms, says Dr. Kramer. Other early symptoms are unexplained fever and pain, swelling, and stiffness of multiple joints. Complications such as inflammation of the lining surrounding the lungs or heart can also occur early on, he adds.
Inflammation of the lining surrounding the lungs, or pleuritis, can occur in people with lupus. This can cause symptoms such as chest pain and shortness of breath, says Luk. The pain can worsen when taking a deep breath, sneezing, coughing, or laughing. (18) Pleural effusion, or fluid around the heart and lungs, may also develop and can cause shortness of breath or chest pain, says Caricchio.

Some patients with mild lupus, with a little joint pain or rash can be managed with anti-inflammatory drugs such as nonsteroidal anti-inflammatory drugs, or NSAIDs, such as ibuprofen or naproxen, says Stuart D. Kaplan, MD, the chief of rheumatology at South Nassau Communities Hospital in Oceanside, New York. These drugs can also help manage fever and inflammation of the heart and lining around the lungs. Elevated expression of HMGB1 was found in the sera of people and mice with systemic lupus erythematosus, high mobility group box 1 (HMGB1) is a nuclear protein participating in chromatin architecture and transcriptional regulation. Recently, there is increasing evidence HMGB1 contributes to the pathogenesis of chronic inflammatory and autoimmune diseases due to its inflammatory and immune stimulating properties.[69]

“My message to patients is that we can do an excellent job of managing the condition compared to 20 years ago,” says Roberto Caricchio, MD, the interim section chief of rheumatology at Temple University Hospital in Philadelphia and the director of the Temple Lupus Clinic at the Lewis Katz School of Medicine. With that said, people should never underestimate the serious effects lupus can have, he adds, which is why working with your doctor to manage the condition is so important.
